THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be amended to add the Legion of Merit and the Army Superior Unit Award and to add skill identifiers (SIs) 7T and 9B to item 11 (Primary Specialty) of her DD Form 214. 2. The applicant states that she received the Legion of Merit after she retired. 3. The applicant provides a DA Form 638 (Recommendation for Award); Legion of Merit orders/certificate; a memorandum, dated 29 July 2004; and her Officer Record Brief, dated 9 August 2004. CONSIDERATION OF EVIDENCE: 1. The applicant was commissioned as a captain in the U. S. Army Reserve, Army Nurse Corps, on 3 September 1981. She entered active duty on 29 October 1981. She was promoted to lieutenant colonel in the Army Nurse Corps on 1 December 1992. 2. The applicant was assigned to Company C, Walter Reed Army Medical Center from on or about 22 June 2000 through 31 October 2004. 3. North Atlantic Regional Medical Command memorandum, dated 29 July 2004, noted that the Army Superior Unit Award was awarded to personnel assigned to listed Walter Reed Army Medical Center units during the period 1 September 2002 to 1 September 2003. Company C was one of the listed units. 4. United States Army Medical Command Permanent Orders 296-17, dated 22 October 2004, awarded the applicant the Legion of Merit. 5. The applicant’s Officer Record Brief shows the applicant held SIs 5K, 7T, and 9B. 6. On 31 October 2004, the applicant was released from active duty due to having sufficient service for retirement. 7. The applicant’s DD Form 214 does not show she was awarded the Legion of Merit or authorized the Army Superior Unit Award. 8. Item 11 of the applicant’s DD Form 214 lists her primary specialty as “66H 5K.” 9. The U. S. Army Human Resources Command’s Officer Area of Concentration Structure Chart, as of 1 January 2008, identifies SI 5K as Instructor; SI 7T as Clinical Nurse Specialist; and SI 9B as Medical Proficiency. SI 9B is also identified as authorized for Medical Corps, Dental Corps, and Army Medical Specialist Corps officers only. 10. On 26 February 2008, the Deputy Chief, Army Nurse Corps, U. S. Army Human Resources Command informed the Board analyst that she could not recall SI 9B ever pertaining to the Army Nurse Corps officers. DISCUSSION AND CONCLUSIONS: 1. The applicant was assigned to Company C, Walter Reed Army Medical Center during a period of time that unit was awarded the Army Superior Unit Award. This unit award should be added to her DD Form 214. 2. The applicant was awarded the Legion of Merit shortly before she retired. This award should be added to her DD Form 214. 3. Item 11 of the applicant’s DD Form 214 shows she held SI 5K. Her Officer Record Brief shows she held, in addition to SI 5K, SIs 7T and 9B. 4. However, SI 9B is not listed in the U. S. Army Human Resources Command’s Officer Area of Concentration Structure Chart as being authorized for Army Nurse Corps officers. Information received from the Deputy Chief, Army Nurse Corps, U. S. Army Human Resources Command indicated that SI 9B never pertained to the Army Nurse Corps. 5. In the absence of evidence to the contrary, it appears that the applicant’s Officer Record Brief erroneously shows she held SI 9B. However, SI 7T should be added to item 11 of her DD Form 214. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. amending item 11 of her DD Form 214 to show her primary specialty as “66H 5K 7T”; and b. adding the Legion of Merit and Army Superior Unit Award to her DD Form 214. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to adding skill identifier 9B to item 11 of her DD Form 214. __James B.
