Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty), to show he served in Saudi Arabia from 29 September 1990 to 28 March 1991 during the Gulf War and in Iraq from 7 August 2005 to 9 September 2005 during “Warfighter Excon.” 2. The applicant states, in effect, that his service in Saudi Arabia and Iraq was not shown due to an administrative oversight. 3. The applicant provided the following documentary evidence in support of his application: a. Headquarters, Fort Stewart and Hunter Army Airfield, Fort Stewart, Georgia, Orders 215-362, dated 3 August 2005, Temporary Change of Station Orders. b. Port Call Instructions, dated 27 July 2005. c. DD Form 1610 [Request and Authorization for Temporary Duty (TDY) Travel for Department of Defense (DOD) Personnel], dated 25 July 2007. d. Memorandum For Record, dated 1 August 2005, Headquarters, 3rd Infantry Division (Mechanized) Artillery, Fort Stewart, Georgia, Authorization for TDY Travel. e. Flight Itinerary, dated 2 August 2005. f. AAA-347 (Enlisted Record Brief), dated 14 November 2000. g. Enlisted Record Brief, dated 3 September 2006. CONSIDERATION OF EVIDENCE: 1. The applicant is a retired master sergeant (MSG)/E-8 who enlisted in the Regular Army on 2 July 1981, served in military occupational specialty 63D (Self-Propelled Field Artillery System Mechanic), and retired on 30 November 2006. 2. Item 5 (Overseas Assignments) of the applicant’s DA Form 2-1 (Personnel Qualification Record) shows that he served in Saudi Arabia from 29 September 1990 to 28 March 1991. 3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), of the applicant's DD Form 214, dated 30 November 2006, shows that the applicant was awarded, among other awards, the Southwest Asia Service Medal, with one bronze service star. 4. The applicant's records show that he served in Kuwait/Iraq during the period 1 September 2002 to 26 August 2003. 5. The applicant provided a copy of Order 215-362, dated 3 August 2005, issued by Headquarters, Fort Stewart and Hunter Army Airfield, Fort Stewart, Georgia. This order directed a temporary change of station to Kuwait/Iraq for a period of 33 days from 7 August 2005 to 9 September 2005 in support of the 3rd Infantry Division exercise called “Warfighter Excon.” 6. The applicant submitted copies of his travel orders and itinerary as well as a Memorandum For Record, dated 1 August 2005, Headquarters, 3rd Infantry Division (Mechanized) Artillery, Fort Stewart, Georgia, authorization for TDY Travel, that show he was reassigned and/or deployed to Kuwait/Iraq for a period of 33 days from 7 August 2005 to 9 September 2005 in support of the 3rd Infantry Division Commanding General exercise called “Warfighter Excon.” 7. Item 18 (Remarks) of the applicant's DD Form 214 does not show his service in Southwest Asia from 29 September 1990 to 28 March 1991 or his service in Kuwait/Iraq from 7 August 2005 to 9 September 2005. However, Item 18 of the applicant’s DD Form 214 shows he served in a designated danger pay area “Kuwait/Iraq from 1 September 2002 to 26 August 2003.” 8. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation directs, in pertinent part, that the purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. Item 13 of the DD Form 214 will reflect decorations, medals, badges, citations and campaign ribbons awarded or authorized for all periods of service. It also states, in pertinent part, that for an active duty Soldier deployed with his or her unit during the period covered by the DD Form 214, the following statement will be entered in Item 18 (Remarks): "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. Evidence of record shows that the applicant served in Saudi Arabia, during the Gulf War, from 29 September 1990 to 28 March 1991. Therefore, he is entitled to correction of his records to show the entry "Service in Southwest Asia From 29 September 1990 to 28 March 1991” in Item 18 of his DD Form 214. 2. Evidence of record further shows that the applicant also served in Kuwait/Iraq for a period of 33 days, from 7 August 2005 to 9 September 2005. Therefore, he is entitled to correction of his records to show the entry “Service in Kuwait/Iraq from 7 August 2005 to 9 September 2005” In 18 of his DD Form 214. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the entries "SERVICE IN SOUTHWEST ASIA 19900929 TO 19910328” and "SERVICE IN KUWAIT/IRAQ FROM 20050807 TO 20050908” to Item 18 (Remarks) of the applicant's DD Form 214, dated 30 November 2006.
