Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that Item 11 (Primary Specialty) and Item 14 (Military Education) of his separation document (DD Form 214) be corrected. 2. The applicant states, in effect, that Item 11 should show that he served in military occupational specialty (MOS) 91S (Preventive Medicine Specialist) for 8 years instead of 5 years, and that Item 14 should include his completion of MOS 13B (Field Artilleryman) military education. 3. The applicant provides completion certificates from MOS 13B One Station Unit Training and from the MOS 91S preventive medicine course in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant's record shows that he initially enlisted in the Regular Army and entered active duty on 18 January 1995. He completed MOS 13B OSUT at Fort Sill, Oklahoma, on 3 May 1995. 2. On 11 December 1998, the applicant completed the MOS 91S preventive medicine course at Fort Sam Houston, Texas, and was awarded MOS 91S. 3. On 30 June 2006, the applicant was honorably released from active duty after completing a total of 11 years, 5 months, and 13 days of active duty training. Item 11 of the DD Form 214 he was issued at the time contains entries indicating he served in MOS 13B for 5 years and 4 months and that he served in MOS 91S for 5 years and 8 months. Item 14 contains entries indicating he completed the Primary Leadership Development Course in 2002 and the 10-week Preventative Medicine Course in 1995. 4. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214. Chapter 2 of the separation documents regulation contains item-by-item preparation instructions for the DD Form 214. 5. The instructions for Item 11 contained in the separation documents regulation state to enter the titles of all MOSs served in for at least 1 year and include the number of years and months served. It further states that for time determination, 16 days or more count as a month, and it stipulates that basic training and AIT should not be counted. The instructions for Item 14 state to list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. It further states that military education information is used to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that Item 11 of his DD form 214 should show he served in MOS 91S for 8 years, and that Item 14 should show his completion of the 13B course was carefully considered and found to have partial merit. 2. The evidence of record confirms the applicant completed the MOS 91S course on 11 December 1998, and that he served in that MOS until his REFRAD on 30 June 2006. Therefore, the 91S portion of the entry in Item 11 should be corrected to show he served in that MOS for 7 years and 7 months (11 December 1998-30 June 2006). 3. The applicant's contention that Item 14 should contain an entry showing his completion of MOS 13B training was also carefully considered. However, by regulation, training courses for combat skills will not be entered in Item 14. As a result, given training in MOS 13B is basically for combat skills related to being a field artilleryman, there is an insufficient evidentiary basis to support granting this portion of the requested relief. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending Item 11 of his DD Form 214 by deleting the current time served in MOS 91S (5 years, 8 months) and replacing it with the entry "7 years, 7 months"; and by providing him a correction to the separation document that reflects this change. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
