THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect that item 12d (Total Prior Active Service) of his DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 18 February 2004, be corrected to show his total prior service as reflected in his previous DD Form 214, dated 1 October 2002. He also requests correction of his DD Form 214 to show the Joint Service Achievement Medal. 2. The applicant states, in effect, that his DD Form 214, dated 18 February 2004, does not show his total prior active service and the Joint Service Achievement Medal. 3. The applicant provides two copies of his DD Form 214, dated 18 February 2004 and 1 October 2002, and an award certificate and orders awarding him the Joint Service Achievement Medal in support of his request. CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a warrant officer one (WO1/W-1) effective 7 July 1995, with prior service. He was promoted to chief warrant officer two (CW2/W-2) effective 8 July 1997. 2. The applicant was ordered to active duty on 28 February 2001 and was released from active duty on 1 October 2002. He had completed 1 year, 7 months, and 4 days of net active service. 3. Item 12c (Net Active Service This Period), of his DD Form 214, dated 1 October 2002, shows the entry "0001 07 04" (1 year, 7 months, and 4 days), item 12d (Total Prior Active Service) shows the entry "0006 10 26" (6 years, 10 months, and 26 days), and item 12e (Total Prior Inactive Service) shows the entry "0011 05 25" (11 years, 5 months, and 25 days). 4. On 20 November 2002, Defense Intelligence Agency, Military Personnel Awards Order Number 324-4 awarded the applicant the Joint Service Achievement Medal for the period 1 March 2001 to 30 September 2002. 5. The applicant was ordered to active duty on 19 February 2003 in support of Operation Enduring Freedom (OEF) for 365 days unless extended or terminated by proper authority. 6. The applicant was promoted to chief warrant officer three (CW3/W-3) effective 7 July 2003. 7. The applicant was released from active duty on 18 February 2004. He was transferred to the United States Army Reserve (USAR) Control Group (IMA [Individual Mobilization Augmentee]). He completed 1 year of net active service. 8. Item 12c (Net Active Service This Period), of his DD Form 214, dated 18 February 2004, shows the entry "0001 00 00" (1 year), item 12d and 12e show the entry "See Block #18." Item 18 (Remarks), of his DD Form 214, indicates the applicant was separated on temporary records and Soldier's affidavit. A DD Form 215 (Correction to DD Form 214) it states, would be issued to provide missing information. 9. The applicant was ordered to active duty on 12 January 2008 for a period of 11 months and 30 days for the purpose of participating in Contingency Operation for Active Duty Operational Support (CO-ADOS) in support of OIF (Operation Iraqi Freedom), with duty at MacDill Air Force Base, Florida. 10. Army Regulation 635-5 governs the preparation of the DD Form 214. It states, in pertinent part, that item 12c will be completed to show the amount of net active service, computed by subtracting item 12a (Date Entered AD This Period) from 12b (Separation Date This Period), less lost time, if any. Item 12d (Total Prior Active Service) will be completed to show the amount of prior active military service, less lost time, if any, and item 12e (Total Prior Inactive Service) will be completed to show the amount of total prior inactive service, less lost time, if any. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows the applicant was awarded the Joint Service Achievement Medal. He is therefore entitled to correction of his DD Form 214, dated 18 February 2004, to show this award. 2. The DD Form 214 issued to the applicant on his release from active duty on 1 October 2002 shows he had completed 1 year, 7 months, and 4 days of net active service, 6 years,10 months, and 26 days of total prior active service, and 11 years, 5 months, and 25 days of total prior inactive service. 3. The DD Form 214 issued to the applicant on his release from active duty on 18 February 2004 indicated in item 12d and 12e the entry "See Block 18." Item 18 (Remarks), of his DD Form 214 indicated that he was separated on temporary records and Soldier's affidavit and that a DD Form 215 would be issued to provide missing information. 4. By adding the applicant's net active service of 1 year, 7 months, and 4 days and his total prior active service of 6 years, 10 months, and 26 days, which equates to 8 years and 6 months, from his DD Form 214, dated 1 October 2002, this total now becomes the applicant's total prior active service for entry in item 12d, of his DD Form 214, dated 18 February 2004. He is therefore entitled to correction of item 12d (Total Prior Active Service), of his DD Form 214, dated 18 February 2004, to show the entry "0008 06 00" (8 years and 6 months). 5. The applicant served in an inactive status in the USAR from 2 October 2002 to 18 February 2003 which equated to 4 months and 17 days of inactive service. By adding the 4 years and 17 days of inactive service to his inactive service of 11 years, 5 months, and 25 days, as shown on his DD Form 214, dated 1 October 2002, which equates to 11 years, 10 months, and 12 days of prior inactive services, this now becomes the applicant's total prior inactive service for entry in item 12e, of his DD Form 214, dated 18 February 2004. He is therefore entitled to correction of item 12e (Total Prior Inactive Service), of his DD Form 214, dated 18 February 2004, to show the entry "0011 10 12" (11 years, 10 months, and 12 days).
