THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 be corrected to reflect her award of the Defense Meritorious Service Medal (DMSM). 2. The applicant states, in effect, that her award of the DMSM awarded to her at the time of her retirement was never added to her DD Form 214. 3. The applicant provides a copy of her DD Form 214; a copy of her award of the DMSM; and a copy of a memorandum from the transition center at Fort Knox, Kentucky requesting that the award of the DMSM be added to the applicant’s DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in St. Louis, Missouri on 26 September 1983 for a period of 4 years and training as a finance specialist. She successfully completed her training and remained on active duty through a series of continuous reenlistments. She was promoted to the pay grade of E-7 on 1 May 2000. 2. On 4 October 2005, the applicant was awarded the Defense Meritorious Service Medal for exceptionally meritorious service from November 2003 to April 2006. The award was a retirement award. 3. On 31 March 2006, the applicant was honorably released from active duty (REFRAD) and was transferred to the Retired List effective 1 April 2006. Her DD Form 214 issued at the time of her REFRAD did not include her award of the DMSM. 4. A review of the applicant’s official records shows that the award of the DMSM was filed in her records along with a copy of the memorandum from the transition center requesting that a DD Form 215 be issued to reflect the award. However, there is no evidence to show that a DD Form 215 was ever issued. DISCUSSION AND CONCLUSIONS: 1. The evidence of record clearly shows that the applicant was awarded the DMSM at the time of her retirement and that the Chief, Transition Center properly requested that a DD Form 215 be issued to reflect that award. 2. However, it appears that an administrative oversight occurred which resulted in the award of the DMSM being filed in her records and no DD Form 215 being issued. 3. Accordingly, it would be in the interest of justice at this time to issue her a DD Form 215 to reflect her award of the DMSM.
