THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that item 12a (Date Entered AD [Active Duty] This Period) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 13 May 2006 be corrected to show 2005 instead of 2006. 2. The applicant states that the year is wrong in item 12a on her DD Form 214 and it needs to be corrected to 2005. She contends that she has tried for the past year and half to get this error corrected through her Brigade and Division with no results or indication that anyone is working on the problem. 3. The applicant provides mobilization orders, dated 20 January 2005; amendment orders, dated 21 January 2005; deployment orders, dated 21 January 2005; a memorandum, dated 10 February 2006; discharge orders, dated 13 April 2006; and a copy of her DD Form 214. CONSIDERATION OF EVIDENCE: 1. Having prior service, the applicant was ordered to active duty from the U.S. Army Reserve on 25 January 2005 in support of Operation Enduring Freedom. She served in Germany from 17 February 2005 to 1 April 2006. On 13 May 2006, she was released from active duty. 2. Item 12a on the applicant’s DD Form 214 shows the entry, “2006 01 25.” Item 12b (Separation Date This Period) on her DD Form 214 shows the entry, “2006 05 13.” Item 12c (Net Active Service This Period) on her DD Form 214 shows the entry, “0001 03 19.” 3. The applicant provided mobilization orders, dated 20 January 2005, which show she was ordered to active duty for a period of 545 days. Her reporting date was 25 January 2005. These orders were amended on 21 January 2005 to show a reporting date of 30 January 2005. DISCUSSION AND CONCLUSIONS: Since orders show the applicant was ordered to active duty in January 2005, it appears item 12a on her DD Form 214 is incorrect. Therefore, item 12a on her DD Form 214 should be corrected to show she entered active duty on 25 January 2005.
