THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that her 25 December 2004 DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to delete the period of service covered by her 10 July 2003 DD Form 214. 2. The applicant states she had enlisted service from May 1995 through July 2003 and was honorably discharged to accept a commission. Her second DD Form 214 incorrectly includes her earlier period of service. 3. The applicant provides copies of her two DD Forms 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 26 May 1995, attained the rank of staff sergeant, and was honorably discharged on 10 July 2003 to accept a commission as a second lieutenant in the Regular Army Nurse Corps. 2. On 25 December 2004, she was released from active duty and placed on the Temporary Disability Retired List (TDRL). 3. The DD Form 214 issued at that time listed her total period of service as a single period of commissioned service from 26 May 1995 through 25 December 2004. 4. The applicant was subsequently found fit for duty and has been recalled to active duty. 5. Army Regulation 635-5 (Separation Documents) states that a DD Form 214 will be prepared for all active Army Soldiers on termination of active duty by reason of administrative separation (including separation by reason of retirement or expiration term of service, physical disability separation, or termination of enlisted status to accept a commission). DISCUSSION AND CONCLUSIONS: 1. In accordance with regulations, the applicant was properly discharged and issued a DD Form 214 on 10 July 2003 when she accepted a commission in the Army Nurse Corps. 2. The applicant's 25 December 2004 DD Form 214 incorrectly lists her total period of service as one continuous period of commissioned service. This administrative error should be corrected to avoid confusion of the applicant's status and prevent further problems as they relate to the time frames involved for her promotion consideration and advancement potential. Therefore, her 25 December 2004 DD Form should be corrected at this time to reflect the entries shown below.
