THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Combat Medical Badge, Army Good Conduct Medal, two Army Achievement Medals, two Army Commendation Medals, Air Assault Badge, and Driver and Mechanic Badge. 2. The applicant states they were left off of his DD Form 214. 3. The applicant provides a copy of a 14 August 2007 memorandum for record (MFR) from an Army psychiatrist in which the doctor states “He has also earned the Combat Medic Badge.” CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 2 February 2005. He completed training and was awarded military occupational specialty 68W (Health Care Specialist). He was retired by reason of temporary disability on 27 July 2008 and placed on the Temporary Disability Retired List (TDRL) because of post-traumatic stress disorder (PTSD) and lumbago. 2. The applicant’s DD Form 214 shows the following awards: National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with bronze service star, Army Service Ribbon, and Overseas Service Ribbon. None of the requested awards are shown. 3. There are no orders in the applicant’s Integrated Personnel Electronic Records Management System (iPERMS) records awarding him any of the requested medals and badges. The applicant has provided no orders showing his entitlement to the requested awards and badges. 4. The applicant’s iPERMS file contains a 3 March 2008 MFR from his company commander disqualifying him for award of the Army Good Conduct Medal because of Uniform Code of Military Justice action under Article 15. 5. Army Regulation (AR) 600-8-22 (Military Awards) sets forth Department of the Army criteria, policy and instructions concerning individual military awards, the Good Conduct Medal, service medals and service ribbons, combat and special skill badges and tabs, unit decorations, and trophies and similar devices awarded in recognition of accomplishments. It provides that all personal decorations require formal recommendations, approval through the chain of command, and announcement in orders. DISCUSSION AND CONCLUSIONS: 1. The applicant requests the Combat Medical Badge, Army Good Conduct Medal, two Army Achievement Medals, two Army Commendation Medals, Air Assault Badge, and Driver and Mechanic Badge. 2. All of the requested awards and badges require announcement in orders. The applicant’s iPERMS records contain no orders, only an MFR disqualifying him for award of the Army Good Conduct Medal. The applicant has not provided copies of orders awarding him any decorations or badges. 3. In order to justify correction of a military record, the applicant must show to the satisfaction of the Board, or it must otherwise satisfactorily appear, that the record is in error or unjust. The applicant has failed to submit evidence that would satisfy this requirement.
