THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show all of the periods he served in designated imminent danger pay (IDP) areas.

2.  The applicant states his DD Form 214 correctly shows his service in Egypt and Iraq.  However, he also served in Saudi Arabia, Kuwait, and Iraq during Operation Desert Shield/Desert Storm and in Iraq during Operation Iraqi Freedom. 

3.  The applicant provides, in support of his request, copies of his DD Form 214, a 1 October 2007 memorandum showing his 2006-2008 IDP service in Iraq, an undated 178th Personnel Service Company memorandum showing his Operation Desert Storm IDP time from 10 December 1990 to 28 April 1991, III Corps and Fort Hood Orders 284-0101 retirement orders, and a National Personnel Records Center extract from the "Gulf War Roster."  

CONSIDERATION OF EVIDENCE:

1.  The applicant retired from the Regular Army as a command sergeant major (E-9) on 31 August 2008.  He had 30 years, 2 months and 10 days of continuous active duty service.



2.  Block 18 (Remarks) of his DD Form 214 states, in part, "SERVED IN IMMINENT DANGER PAY AREA//SERVICE in Egypt 19960706-19990716//SERVICE IN IRAQ 20040312-20050311//."

3.  The applicant submitted:

   a.  an undated 178th Personnel Service Company memorandum showing he  served during Operation Desert Storm from 10 December 1990 to 28 April 1991, received IDP, and was authorized to wear a wartime service sleeve insignia; and 
   
   b.  a 1 October 2007 memorandum from 1st Battalion, 8th Cavalry Regiment, 2nd Brigade, 1st Cavalry Division showing he served in Iraq during Operation Iraqi Freedom from 23 October 2006 to 6 January 2008.

4.  Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in block 18.

DISCUSSION AND CONCLUSIONS:

1.  The applicant states his DD Form 214 correctly shows his service in Egypt and Iraq.  However, he also served in Saudi Arabia, Kuwait, and Iraq during Operation Desert Shield/Desert Storm and in Iraq during Operation Iraqi Freedom. 

2.  The 178th Personnel Service Company memorandum shows he served in an IDP area during Operation Desert Storm from 10 December 1990 to 28 April 1991.

3.  The 1 October 2007 memorandum shows he served in an IDP area in Iraq during Operation Iraqi Freedom from 23 October 2006 to 6 January 2008.

4.  The above service should be added to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to block 18 of his DD Form 214, "SERVICE IN SAUDI ARABIA-KUWAIT-IRAQ 19901210-19910428//SERVICE IN IRAQ 20061023-20080106//."
