It is important that information entered on the form be complete and accurate. This regulation specifies that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. The DD Form 214 is prepared for all personnel at the time of their retirement, discharge, or release from active duty. Item 11 of the DD Form 214 reflects military occupational specialties. This item contains titles of all MOS's held or served for at least 1 year and includes for each MOS the number of years and months served. The regulation provides information found in item 11 will be derived from the Enlisted Record Brief. DISCUSSION AND CONCLUSIONS: The applicant held MOS 88M2O and MOS 11B2O from 1 December 2000. When he served on active duty, he still held those two specialties. They should both be added to item 11 of his DD Form 214 and the "years and months held" should be entered as 3 years and 5 months (from 1 December 2000 to 20 April 2004).
