THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Good Conduct Medal (AGCM). 2. The applicant states she received the AGCM but is not listed on her DD Form 214. 3. The applicant provides an AGCM Certificate in support of this application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 23 June 2005. She completed initial entry training and was awarded military occupational specialty 92A (Automated Logistical Specialist). 2. She provides an AGCM Certificate that shows she was awarded the AGCM by Permanent Orders Number 362-001, issued by the Brigade Support Battalion, FOB Hammer, Iraq. 3. On 4 June 2009, she was honorably discharged by reason of physical disability after completing 3 years, 11 months, and 12 days of active military service. Item 13 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214 shows she earned the following awards: * Army Commendation Medal * Army Achievement Medal (2nd award) * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * Iraq Campaign Medal (ICM) with one bronze service star 4. Item 18 (Remarks) of her DD Form 214 shows she served in Iraq from 4 October 2006 to 19 December 2007. 5. Army Regulation 600-8-22 (Military Awards) states the ICM is awarded to members who have served in direct support of Operation Iraqi Freedom. It further states a bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003-1 May 2003) * Transition of Iraq (2 May 2003-28 June 2004) * Iraqi Governance (29 June 2004-15 December 2005) * National Resolution (16 December 2005-9 January 2007) * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-date to be determined) DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that her DD Form 214 should be corrected to show the AGCM has been carefully reviewed. 2. Permanent orders awarded her the AGCM. Therefore, she is entitled to correction of her DD Form 214 to show this award. 3. Records show she was awarded the ICM with one bronze service star; however, she participated in two campaigns while in Iraq (National Resolution and Iraqi Surge). Therefore, she is also entitled to correction of her DD Form 214 to show the ICM with two bronze service stars.
