THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his first name as "JERRAD" instead of "JARRED." 2. The applicant states his first name is incorrectly spelled on his DD Form 214. 3. The applicant provides his DD Form 214 and his birth certificate. CONSIDERATION OF EVIDENCE: 1. The applicant's records show that on 9 January 2006, a DD Form 369 (Police Record Check) was submitted by the U.S. Army Recruiting Station, La Crosse, WI, wherein his first name is spelled as "JERRAD." He authenticated this form by signing in the appropriate place using the first name spelling of "JERRAD." 2. On 11 January 2006, he underwent a medical examination at the Milwaukee Military Entrance Processing Station (MEPS). The DD Form 2808 (Report of Medical Examination) lists his first name as "JERRAD." 3. On 17 July 2006, he enlisted in the Regular Army (RA) for 3 years at the Milwaukee MEPS. The DD Form 4 (Enlistment/Reenlistment Document of the Armed Forces of the United States) lists his first name as "JERRAD." A DD Form 1966 (Record of Military Processing - Armed Forces of the United States) was completed on the same date wherein his first name is spelled as "JARRED." He authenticated both forms by signing his name in the appropriate place using the first name spelling of "JERRAD." 4. His records contain several personnel, finance, and legal documents including a record of Servicemen's Group Life Insurance Election, assignment orders, and various other orders that show his first name spelling as "JARRED." On each form requiring a signature, he authenticated the form by placing his signature in the appropriate block using the first name spelling of "JERRAD." 5. On 20 October 2009, he filled out a DD Form 2807-1 (Report of Medical History) wherein he indicated his first name as "JERRAD." 6. He was honorably discharged from active duty on 27 January 2010. The DD Form 214 he was issued shows his first name spelling as "JARRED." 7. He submitted his birth certificate, issued 9 January 2006, which shows his first name spelling as "JERRAD." DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that his first name shown on his DD Form 214 should be corrected to show his first name spelling as "JERRAD" was carefully considered. 2. The available evidence shows that prior to and upon enlistment into the RA on 19 July 2006 he listed his first name spelling as "JERRAD." However, it appears that during processing at the MEPS on the same date, the letters "E" and "A" in his first name were inadvertently transposed and his first name was listed as "JARRED." Throughout his military service he consistently signed his first name as "JERRAD," although documents showed his first name spelling as "JARRED." 3. In view of the foregoing, he is entitled to correction of his DD Form 214 to show his first name spelling as "JERRAD."
