THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction to items 12d (Total Prior Active Service), item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), item 14 (Military Education), and item 18 (Remarks) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 29 March 2010 2. She states she had 4 years, 4 months, and 12 days of prior active service in the U.S. Air Force (USAF). 3. The applicant provides her DD Forms 214 for the period ending 1 February 2010 and 29 March 2010. CONSIDERATION OF EVIDENCE: 1. The applicant record shows she served on active duty in the USAF for a period of 4 years, 4 months, and 12 days from 20 September 2005 through 1 February 2010, at which time she was honorably discharged, in the rank of senior airman (SRA)/E-4, for miscellaneous/general reasons. 2. The DD Form 214 she was issued upon her discharge from the USAF shows in: a. item 12c (Net Active Service This Period) the entry "04 04 12"; b. item 13 the: * Air Force Achievement Medal * Meritorious Unit Award * Air Force Outstanding Unit Award (1st Oak Leaf Cluster) * Air Force Good Conduct Medal * National Defense Service Medal * Iraq Campaign Medal with 1 bronze service star * Global War on Terrorism Service Medal * Air Force Overseas Ribbon Short * Air Force Overseas Ribbon Long * Air Force Expeditionary Service Ribbon with Gold Border * Air Force Longevity Service * Air Force Training Ribbon c. Item 14 the entry "Supply Management Apprentice, Jan 2006; Basic Military Training, Nov 2005"; and d. item 18 the entry "MEMBER HAS COMPLETED FIRST FULL TERM OF SERVICE." 3. The applicant enlisted in the Regular Army (RA) for a period of 4 years in the rank/grade of specialist (SPC)/E-4. On 3 February 2010, she was discharged with an uncharacterized character of service in the rank/grade of SPC/E-4. 4. The DD Form 214 she was issued upon her discharge from the RA shows in: a. item 12d, the entry "0000 00 00"; b. item 13, the entry "None"; c. item 14, the entry "None"; and d. item 18, the entry "Member has not completed first full term of service." 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains item-by-item preparation instructions for the DD Form 214. The instructions state for: a. item 12d, from previously-issued DD Forms 214, enter the total amount of prior active military service less time lost, if any. If not applicable, enter "00 00 00" b. item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards). Each entry will be verified by the Soldier’s records. Do not use abbreviations. c. item 14, from the Enlisted Record Brief (ERB), list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. d. item 18, enter the mandatory entry: "SOLDIER (HAS) (HAS NOT) COMPLETED FIRST FULL TERM OF SERVICE." It also states it is also used to add Headquarters, Department of the Army (HQDA) requirements for which a separate block is not available. DISCUSSION AND CONCLUSIONS: 1. The applicant’s request to correct item 12d, 13, and 18 to properly document her USAF service has been carefully considered and found to have merit. 2. In this case, the evidence of record confirms the applicant completed 4 years, 4 months, and 12 days of prior active military service in the USAF. Therefore, item 12d of her RA DD Form 214 for the period ending 29 March 2010 should be corrected to show her total prior active service. 3. The DD Form 214 the applicant was issued upon her discharge from the USAF confirms she earned several awards during that period of service. Therefore, it would be appropriate to correct her DD Form 214 for the period ending 29 March 2010 to show the same awards as listed on her USAF DD Form 214 for the period ending 1 February 2010. 4. The record also confirms the applicant completed her first full term of service in the USAF on 1 February 2010. Therefore, in the interest of equity her DD Form 214 for the period ending 29 March 2010 should be corrected to show she has completed her first full term of service.” In addition, the entry “UNITED STATES AIR FORCE ACTIVE SERVICE FROM 20050920 - 20100201” should be entered in item 18 of her DD Form 214 for the period ending 29 March 2010 in the interest of clarity. 5. The applicant’s request to correct item 14 of her DD Form 214 for the period ending 29 March 2010 to add her military education completed in the USAF has also been carefully considered. However, by regulation, only formal in-service training courses successfully completed during the period of service covered by the DD Form 214 will be entered. There is also no evidence the applicant completed any courses during the period covered by her RA DD Form 214. Therefore, there is no basis for granting this portion of her request. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting from item 12d of her DD Form 214 for the period ending 29 March 2010 the entry "0000 00 00" and replacing it with the entry “0004 04 12”; b. deleting from item 13 of her DD Form 214 for the period ending 29 March 2010 the entry "None" and replacing it with the list of awards contained on her DD Form 214 for the period 1 February 2010 issued for her USAF active duty service; c. deleting from item 18 of her DD Form 214 for the period ending 29 March 2010 the entry "MEMBER HAS NOT COMPLETED FIRST FULL TERM OF SERVICE" and replacing it with the entry “MEMBER HAS COMPLETED FIRST FULL TERM OF SERVICE”; and d. adding to item 18 of her DD Form 214 for the period ending 29 March 2010 the entry “UNITED STATES AIR FORCE ACTIVE DUTY SERVICE FROM 20050920 - 20100201.” 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
