THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his Survival, Evasion, Resistance, and Escape (SERE) High Risk training and foreign service in support of Operation Enduring Freedom - Philippines. 2. The applicant states he completed SERE training and served in a designated imminent danger pay area in Operation Enduring Freedom – Philippines on numerous occasions. 3. The applicant provides: * DD Form 214 * DA Form 4037 (Officer Record Brief) * Soldier Deployment History Outprocessing Report * ALARACT 259/2005 * MILPER Message 09-026 * two memoranda * SERE High Risk Training Diploma * Temporary Change of Station Orders * 16 pages of Temporary Duty (TDY) Orders and Amendments * DD Form 1351-2 (Travel Voucher or Subvoucher) * DD Form 1351 (Travel Voucher) CONSIDERATION OF EVIDENCE: 1. Having had prior enlisted service, the applicant's records show he was appointed as a warrant officer (WO) and he executed a DA Form 71 (Oath of Office - Military Personnel) on 27 March 1996. He subsequently completed the Special Forces Technical Warrant Officer Basic Course and was assigned to Company B, 3rd Battalion, 1st Special Forces Group, Fort Lewis, Washington. 2. Section VI (Military Education) of the applicant’s DA Form 4037 contains the entry “SERE (High Risk) 1996.” 3. The applicant’s record contains and he also provided SWC FM 2221-7 (Special Warfare Center and School – Diploma) which shows he successfully completed the SERE High Risk training course from 29 July 1996 through 16 August 1996. 4. Section 1 (Assignment Information) of his DA Form 4037 shows the applicant completed deployments to the Republic of the Philippines on: * 17 April 2004 * 15 August 2004 * 23 December 2004 * 25 February 2005 * 17 May 2005 * 7 June 2006 5. The applicant provides a copy of an Army Human Resource System – Outprocessing Report which displays his deployment history. The document shows the applicant deployed in a named operation for the purpose of war from: * 18 February through 14 May 2002 * 14 March through 17 April 2004 * 19 July through 15 August 2004 * 19 through 23 December 2004 * 27 January through 25 February 2005 * 29 March through 17 May 2005 * 19 June through 7 June 2006 6. The applicant provided copies of his DD Form 1610 (Request and Authorization for TDY Travel of Department of Defense Personnel) for the periods: * 14 March through 17 April 2004 * 19 July through 15 August 2004 * 19 through 23 December 2004 * 27 January through 25 February 2005 * 29 March through 8 May 2005 * 19 June through 14 December 2005 * 26 December 2005 through 2 April 2006 7. On 27 May 2011, the Defense Finance and Accounting Service (DFAS) was able to confirm the applicant received hostile fire pay/imminent danger pay and combat zone tax exclusion for service in Afghanistan for the period 18 February 2002 through 28 February 2005. 8. Army Regulation 635-5 (Separation Documents), established the standardized policy for preparing and distributing the DD Form 214. It states, in pertinent part, that the Soldier's military education is obtained from the Soldier's records. It shows the formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 and includes title, length in weeks, and year completed. 9. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter in block 18 the statement "Service in (Name of Country Deployed) From (inclusive dates for example, YYYYMMDD - YYYYMMDD)." 10. Army Regulation 635-5 also prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part, it states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. DISCUSSION AND CONCLUSIONS: 1. The applicant’s request to correct his DD Form 214 to show his SERE High Risk training and foreign service in support of Operation Enduring Freedom – Philippines has been carefully considered and found to have merit. 2. The evidence of record shows the applicant completed the SERE High Risk training course in 1996, which is not shown on his records; therefore, he is entitled to correction of his records to show this course. 3. DFAS was able to confirm he deployed in support of Operation Enduring Freedom for the period 18 February 2002 through 28 February 2005. Additionally, the applicant was able to provide additional evidence which verifies his deployment dates in support of Operation Enduring Freedom - Philippines. Therefore, he is entitled to have these deployments shown in item 18 of his DD Form 214.
