Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests two awards of the Army Achievement Medal (AAM) and two awards of the Army Commendation Medal (ARCOM) be added to her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 30 October 2010. She also requests her DD Form 214 be corrected to show she completed the Army Medical Department (AMEDD) Officer Basic Course and AMEDD Captains Career Course. 2. The applicant states at demobilization she did not have the hard copies with her to verify receipt of the awards and completion of education. 3. The applicant provides: * Permanent orders and DA Forms 638 (Recommendation for Award) for two awards of the AAM * Permanent orders and DA Forms 638 for two awards of the ARCOM * Diploma for the AMEDD Officer Basic Course * Certificate of Completion for the AMEDD Captains Course (6-8-C22) CONSIDERATION OF EVIDENCE: 1. The applicant was ordered to active duty on 7 November 2008 from the U.S. Army Reserve in support of Operation Enduring Freedom. She served as an operating room nurse in Germany from 21 November 2008 to 24 October 2010. On 30 October 2010, she was released from active duty. 2. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of her DD Form 214 for the period ending 30 October 2010 does not show award of the AAM or ARCOM. Item 14 (Military Education) of her DD Form 214 shows the entry "NONE//NOTHING FOLLOWS." 3. She provided a diploma which shows she completed the 2-week AMEDD Officer Basic Course on 16 December 2006. 4. Permanent orders, dated 9 September 2007, show she received the AAM for the period 10 June 2007 to 30 June 2007. 5. Permanent orders, dated 4 May 2008, show she received the ARCOM for exceptional service during the period 14 March 2008 to 20 March 2008. However, the DA Form 638 shows the reason for the award was for achievement. 6. Permanent orders, dated 10 August 2008, show she received the AAM for the period 9 June 2008 to 17 June 2008. 7. She provided a certificate of completion for the AMEDD Captains Career Course (6-8-C22) on 9 June 2009. Research revealed the AMEDD Captains Career Course is a 9-week course. 8. Permanent orders, dated 13 December 2010, show she received the ARCOM for meritorious service during the period 7 November 2008 to 6 November 2010. 9. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214 and states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It states that for: a. item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 will be entered on the DD Form 214. Each entry will be verified by the Soldier’s records. b. item 14, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. Include title, length in weeks, and year completed. DISCUSSION AND CONCLUSIONS: 1. Permanent orders show she received two awards of the AAM and two awards of the ARCOM. Her DD Form 214 for the period ending 30 October 2010 should be corrected to show these medals. 2. The available evidence shows she completed the 2-week AMEDD Officer Basic Course on 16 December 2006. However, the governing regulation states item 14 of the DD Form 214 will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered; therefore, there is no basis for adding this course to her DD Form 214 covering the period of service from 7 November 2008 to 30 October 2010. This portion of her request should be denied. 3. Evidence shows she completed the 9-week AMEDD Captains Career Course on 9 June 2009. Her DD Form 214 for the period ending 30 October 2010 should be corrected to show this course. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. adding two awards of the AAM and two awards of the ARCOM to item 13 of her DD Form 214 for the period ending 30 October 2010; and b. deleting the entry in item 14 of this DD Form 214 and adding the entry "AMEDD Captains Course, 9 Weeks, June 2009." 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
