The applicant requests correction of the DD Form 214 (Report of Separation from the Armed Forces of the United States) of her deceased husband, a former service member (FSM), to show his correct date of birth (DOB). 2. The applicant states the FSM’s correct DOB is October , not December . 3. The applicant provides the FSM's: * DD Form 214 * Birth Certificate from the State of Minnesota * Death Certificate from the State of Minnesota CONSIDERATION OF EVIDENCE: 1. The FSM's military records are not available to the Board for review. A fire destroyed approximately 18 million service members’ records at the National Personnel Records Center in 1973. It is believed that the FSM's records were lost or destroyed in that fire. However, there were sufficient documents remaining in a reconstructed record for the Board to conduct a fair and impartial review of this case. 2. The applicant's DD Form 214 (Report of Separation from the Armed Forces of the United States) shows he was: * born on October * inducted into the Army of the United States on 27 February 1952 * separated on 19 February 1954 3. The applicant submitted a copy of the FSM’s birth certificate and death certificate that shows his DOB as October . DISCUSSION AND CONCLUSIONS: The available evidence is sufficient to reasonably establish that the FSM's DOB is October . Therefore, the FSM's DD Form 214 should be corrected to show his DOB as October .
