THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) as follows: * Item 8a (Last Duty Assignment and Major Command), to show she was assigned to Company A, Army Network Warfare Detachment, U.S. Army Intelligence and Security Command (INSCOM) * Item 18 (Remarks), to add her initial deployment to Afghanistan, from 22 April 2006 to 22 October 2006 2. The applicant states that she has attached evidence in support of her request, including orders, amendments, travel vouchers, award recommendations, and award orders. 3. The applicant provides: * DA Form 4187 (Personnel Action), dated 4 December 2006 * Orders 293-01, U.S. Army INSCOM, dated 20 October 2009 * DD Form 1610 (Request and Authorization for TDY Travel of DOD Personnel), dated 7 April 2006 * DD Form 1610, dated 19 July 2006 * 4 different Request and Travel Authorization/Expense Reports, dated 28 April 2006, 24 July 2006, and 30 October 2006 (2) * Memorandum, Headquarters, Combined/Joint Task Force 76, Bagram Airfield, Afghanistan, dated 30 July 2006 * Permanent Orders 294-044, Headquarters, Combined/Joint Task Force 76, Bagram Airfield, Afghanistan, dated 21 October 2006 * DD Form 2413 (Joint Service Commendation Medal Certificate), dated 21 October 2006 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 1 April 2002. She completed training and was awarded military occupational specialty 35N (Signals Intelligence Analyst). 2. Her Enlisted Records Brief (ERB) documents 3 different deployments: * Korea – from 11 December 2002 through 5 January 2004 * Iraq – from 4 December 2007 through 28 February 2008 * Afghanistan – from 4 April 2008 through 18 May 2008 3. Her DA Form 2166-8 (Noncommissioned Officer (NCO) Evaluation Report), for the period 1 August 2008 through 31 July 2009, shows she was assigned to Company A, Army Network Warfare Battalion, Fort George G. Meade, MD, subordinate to U.S. Army INSCOM. 4. Orders 293-01, U.S. Army INSCOM, dated 20 October 2009, reassigned her to the U.S. Army Transition Point, Fort George G. Meade, MD, for separation processing. These orders show she was assigned to Company A, Army Network Warfare Detachment. 5. On 8 January 2010, she was honorably discharged. Her DD Form 214 shows she was credited with 8 years, 6 months, and 23 days of active service. The highest rank/grade she attained while serving on active duty was staff sergeant/E-6. * Item 8a shows she was assigned to "W001 743 MI BN RPD 2 AS" * Item 18 contains the entries "SERVICE IN IRAQ 20071204 – 20080228" and "SERVICE IN AFGHANISTAN 20080404 – 20080518" 6. According to the U.S. Army Force Management Support Agency (USAFMSA) website, which provides authorization document reference capability based on unit identification code (UIC), the Army Network Warfare Detachment, as it was known at the time in question, was subordinate to the 743rd Military Intelligence Battalion, Fort George G. Meade, MD. 7. The Defense Finance and Accounting Service (DFAS) records show the applicant received hostile fire/imminent danger pay and combat zone tax exclusion for: * Afghanistan – from 21 April 2006 through 21 October 2006 * Iraq – from 5 December 2007 through 2 March 2008 * Afghanistan – from 8 April 2008 through 14 May 2008 8. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active service or control of the Active Army. Chapter 2 contains guidance on the preparation of the DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The preponderance of the evidence of record shows she deployed to Afghanistan on 2 occasions, the first being from 21 April 2006 through 21 October 2006. This initial deployment to Afghanistan is not documented on her DD Form 214; therefore, it would be appropriate to amend item 18 of her DD Form 214 to show this deployment. 2. With respect to her request for correction of her last unit of assignment and major command, the evidence of record shows that at the time of her discharge, she was assigned to Company A, Army Network Warfare Detachment. The evidence further shows this organization was subordinate to the 743rd Military Intelligence Battalion, which is the unit represented on her DD Form 214. The applicant has not shown how this entry has disadvantaged her; therefore, there is an insufficient basis to grant this portion of the requested relief. 3. Despite the findings in paragraph 2 above, item 8a of the applicant's DD Form 214 does not clearly distinguish her last major command. The evidence of record shows that her last unit of assignment was subordinate to U.S. Army INSCOM, which is not shown on her DD Form 214; therefore, it would be appropriate to amend item 8a of her DD Form 214 to show this major command. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. amending item 8a of her DD Form 214 to add "INSCOM" b. amending item 18 of her DD Form 214 to: * delete the entry "SERVICE IN AFGHANISTAN 20080404 – 20080518" * add the entries "SERVICE IN AFGHANISTAN 20060421 – 20061021" and "SERVICE IN AFGHANISTAN 20080404 – 20080518" 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.
