THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected by deleting the Army Staff Identification Badge and adding the Department of Defense Staff Identification Badge and Defense Meritorious Service Medal. 2. The applicant states her DD Form 214 incorrectly reflects that she was awarded the Army Staff Identification Badge and does not show she was awarded the Department of Defense Staff Identification Badge and Defense Meritorious Service Medal. 3. The applicant provides a copy of her DD Form 214 and her authorizations for award of the Department of Defense Staff Identification Badge and the Defense Meritorious Service Medal. CONSIDERATION OF EVIDENCE: 1. The applicant was a Retired sergeant major when she was ordered to active duty on 1 August 2007. She served until she was honorably released from active duty (REFRAD) on 31 July 2008, due to completion of required service. 2. Her DD Form 214 issued at the time of her REFRAD shows she was awarded the Army Staff Identification Badge; however, there is no authorization for that award in her official records. 3. On 7 January 2009, Military Personnel Orders Number 09-002 issued by the Office of the Secretary of Defense awarded the applicant the Defense Meritorious Service Medal for the period of August 2007 to August 2008. 4. On 13 March 2009, a memorandum issued by the Under Secretary of Defense authorized the applicant permanent issue of the Office of the Secretary of Defense Identification Badge for the period of 1 August 2007 to 1 August 2008. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contentions have been noted and appear to have merit. The applicant’s DD Form 214 indicates she was awarded the Army Staff Identification Badge; however, there is no authorization for this award in her OMPF. 2. The applicant contends that she was awarded the Office of the Secretary of Defense Identification Badge and the Defense Meritorious Service Medal and she has provided the appropriate documents to document her authorization for these awards. 3. Accordingly, the award of the Army Staff Identification Badge should be deleted from her DD Form 214 and the Office of the Secretary of Defense Identification Badge and Defense Meritorious Service Medal should be added to her DD Form 214.
