THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show in block 11 (Primary Specialty) the entries “91B4O Health Care Specialist – 9 years, 2 months”; “91W4O Health Care Specialist -4 years, 9 months”; and “91A10 – Medical Specialist – 2 years, 9 months”; in block 14 (Military Education), that the entry “7” in line four be deleted, and in block 18 (Remarks) that the entry “Service in Iraq 20070910 -20091120” be corrected to read “Service in Iraq 20070910 -20081120.” 2. The applicant states his DD Form 214 contains incorrect entries in block 11, 14 and 18. 3. The applicant provides an unofficial copy of his DD Form 2586 (Verification of Military Experience and Training) and a copy of his DD Form 214. CONSIDERATION OF EVIDENCE: 1. After serving in the Virginia Army National Guard from 9 October 1986 to 18 February 1987, he enlisted in the Regular Army on 11 August 1989 for training as a 91A medical specialist. He completed his training at Fort Sam Houston, Texas and was transferred to Germany and his military occupational specialty (MOS) was changed to 91B (Medical Specialist) upon his promotion to the pay grade of E-4. 2. He remained on active duty through a series of continuous reenlistments and was promoted to the pay grade of E-7 on 1 June 1999. He continued to serve as a medical specialist; however, his MOS was changed to 91W in 2003 and in 2006 it was changed to 68W. 3. He served in Iraq from 20070910 – 20081120 and was returned to Fort Riley, Kansas where he remained until he was honorably retired on 30 June 2009 and was transferred to the Retired Reserve effective 1 July 2009. He had served 20 years and 8 months of active service and his DD Form 214 shows in block 11 the entries “91B4O Wheeled Vehicle Mecha – 12 years 4 months” and “91W4O Metal Worker – 3 years 10 months.” In block 14 it contains an entry in line four of “7” and in block 18 it shows he served in Iraq from 20070910 – 20091120. 4. Army Regulation 635-5 serves as the authority for the preparation of the DD Form 214. It provides that information contained on the DD Form 214 will be obtained from official records. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contentions that his DD Form 214 contains errors has been noted and appears to have merit. 2. His DD Form 214 incorrectly reflects in block 11 that MOS 91B is a “Wheeled Vehicle Mechanic” instead of a medical specialist and that MOS 91W is a “metal worker” instead of a Health Care Specialist. Additionally, his service as a 91A medical specialist is not reflected. 3. In block 14, there is an entry at the beginning of line four of “7” which appears to be a typographical error and should be removed. 4. Additionally, in block 18, his DD Form 214 incorrectly reflects that he served in Iraq from 2007 to 2009 instead of 2007 to 2008. Accordingly, that entry should be corrected.
