Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his 3 March 2011 DD Form 214 (Certificate or Release or Discharge from Active Duty) to show award of the Joint Meritorious Unit Award (JMUA) and Armed Forces Service Medal (AFSM). 2. The applicant states he did not have the supporting documentation to show authorization for the awards when he was released from active duty (REFRAD). 3. The applicant provides copies of his Joint Service Achievement Medal award documents, announcement of award of the JMUA, and AFSM award criteria. CONSIDERATION OF EVIDENCE: 1. The applicant had prior honorable enlisted service in the U.S. Army Reserve (USAR) from 5 March 1980 through 3 August 1995. 2. The applicant was appointed as a Reserve warrant officer on 4 August 1995. He was awarded primary specialty 251A0 (Information Systems Technician). 3. He was ordered to active duty in support of Operation Enduring Freedom (OEF) from 15 March 2003 through 17 March 2004 and again from 2 January 2009 through 16 January 2010, during which time he served in Kuwait from 24 January to 13 December 2009. 4. A DD Form 214 shows the applicant was ordered to active duty in support of OEF on 19 February 2010. He served in Haiti from 25 April to 1 June 2010. a. He completed 1 year and 15 days of active service this period that included 1 month and 7 days of foreign service. b. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) in part shows the Joint Service Achievement Medal. It does not show the AFSM or JMUA. 5. In support of his application, the applicant provides the following documents: a. Joint Task Force-Haiti (JTF-H), Port-au-Prince, Haiti, Permanent Orders JTF-H-131-001-10, dated 11 May 2010 and DD Form 2414 (Joint Service Achievement Medal) certificate, dated 11 May 2010, that announced award of the Joint Service Achievement Medal to the applicant for meritorious service in support of Operation Unified Response from 25 April to 1 June 2010. The documents show the applicant was assigned to Headquarters, JTF-H. b. The Joint Staff, Washington, DC, memorandum, dated 22 September 2010, Joint Staff Permanent Order Number J-ISO-0190-10, and Citation accompanying award of the JMUA, that announced award of the JMUA to Headquarters, JTF-H, for meritorious achievement during Operation Unified Response from 14 January to 1 June 2010. c. Under Secretary of Defense, Washington, DC memorandum, dated 28 December 2010, that shows the AFSM was authorized for award to qualifying Service members who deployed to Haiti in support of Operation Unified Response during the period 14 January through 1 June 2010. 6. Army Regulation 600-8-22 (Military Awards) provides policy, criteria, and administrative instructions concerning military awards and decorations. The AFSM may be awarded to members of the Armed Forces of the United States for operations for which no other U.S. campaign or service medal is approved and who, after 1 June 1992: * participate or have participated as members of U.S. military units in a U.S. military operation deemed to be a significant activity and encounter no foreign armed opposition or imminent threat of hostile action * participate for 1 or more days in the operation within the designated area of eligibility or meet one or more of several criteria including being engaged in direct support for: * 30 consecutive days in the area of eligibility (or for the full period when an operation is of less than 30 days' duration) * 60 nonconsecutive days provided this support involves entering the area of eligibility DISCUSSION AND CONCLUSIONS: 1. Records show that during the applicant's assignment to Headquarters, JTF-H, permanent orders awarded the unit the JMUA. 2. Records show the applicant qualified for award of the AFSM for his service in Haiti in support of Operation Unified Response from 25 April to 1 June 2010. 3. In view of the foregoing, it would be appropriate to correct the applicant's 3 March 2011 DD Form 214 to show the awards as recommended below.
