THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his previously-denied request that his Social Security Number (SSN) on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) be changed from “562-xx-xxxx” to “568-xx-xxxx.” 2. The applicant states the incorrect listing of his SSN is causing him difficulty with the Department of Veterans Affairs. 3. The applicant provides: * Special Orders Number 017 * DD Form 214 * a letter from the Social Security Administration * DD Form 256A (Honorable Discharge Certificate) * Orders 12-1213180 * PHS-731 (International Certificates of Vaccination) CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20100025988 on 10 May 2011. 2. The applicant provided his DD Form 256A and PHS-731. These items were not previously reviewed by the ABCMR; therefore, they are considered new evidence and as such warrant consideration by the Board. 3. A review of documents contained in the applicant's military personnel records shows his SSN appears as both "562-xx-xxxx" and "568-xx-xxxx" on several documents dated throughout his tenure of service. 4. The applicant was inducted into the Army of the United States on 19 January 1971. Item 2 (Service Number (SN)) of his DD Form 47 (Record of Induction) lists SSN “568-xx-xxxx.” 5. Special Orders Number 12, issued by the Armed Forces Examining and Entrance Station, Los Angeles, CA, dated 19 January 1971, shows the applicant was inducted into the Army of the United States with SSN “568-xx-xxxx.” 6. On 18 January 1973, the applicant was honorably release from active duty and transferred to the U.S. Army Reserve (USAR) Control Group (Annual Training) to complete his remaining Reserve obligation. The DD Form 214 he was issued at the time shows his SSN as “562-xx-xxxx.” 7. The applicant provides a copy of his discharge orders, Honorable Discharge Certificate, and a letter issued by the Social Security Administration which shows his SSN as "568-xx-xxxx." 8. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. This regulation established standardized policy for the preparation of the DD Form 214. It states the complete name and SSN of the separating service member will be entered on the DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The applicant’s request for reconsideration of his previously-denied request for correction of his SSN on his DD Form 214 has been re-examined. 2. Although the applicant's SSN appears as both "562-xx-xxxx" and "568-xx-xxxx" in his military personnel records, his initial induction documents and the Social Security Administration state the applicant's SSN is "568-xx-xxxx." Therefore, the applicant is entitled to have his DD Form 214 for the period ending 18 January 1973 corrected to show this SSN.
