THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal (ARCOM).

2.  She states the award was never presented prior to her Expiration Term of Service and as a result, was not added to her DD Form 214.  

3.  She provides her a copy of her DD Form 214 and a DA Form 638 (Recommendation for Award). 

CONSIDERATION OF EVIDENCE:

1.  Her records show she enlisted in the Regular Army on 19 April 2007.  After completion of training, she served in military occupational specialty 68W (Healthcare Specialist).

2.  She provided a DA Form 638 for the period 31 December 2008 to 14 April 2011.  Part V (Orders Data) shows Brooke Army Medical Center issued Permanent Order Number 066-03 for award the ARCOM on 7 March 2011. 

3.  She was honorably released from active duty on 18 April 2011 and transferred to the U.S. Army Reserve Control Group (Reinforcement) for completion of her Reserve service obligation.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) does not show the ARCOM. 


DISCUSSION AND CONCLUSIONS:

Permanent orders awarded her the ARCOM.  As a result, she is entitled to have this award added to her DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the ARCOM to item 13 of her DD Form 214.
