THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests all of her military records be changed to reflect her last name as Jxxxxx instead of Dxxxxxx. 2. The applicant states she got married on 25 June 2011 and would like her records to reflect her new last name of Jxxxxx. 3. The applicant provides a marriage license, her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 January 2010, and a DD Form 215 (Correction to DD Form 214). CONSIDERATION OF EVIDENCE: 1. On 17 August 1995, following a period of prior active service, she was appointed as a Reserve commissioned officer in the rank of second lieutenant. She entered active duty on 19 August 1995. 2. She retired from active duty for length of service on 31 January 2010 in the rank of major. a. The DD Form 214 she was issued at the time showed her last name spelled as Dxxxxxx. b. A DD Form 215, dated 4 February 2010, shows her last name spelled as Dxxxxxx. c. Throughout her military records during the above period of active duty her last name is spelled as Dxxxxxx. 3. The applicant provides a copy of a marriage license from the County of Bexar in the State of Texas which shows she married Rxxxx Jxxxxx on 25 June 2011. DISCUSSION AND CONCLUSIONS: 1. The applicant's records show she was appointed and served under the last name of Dxxxxxx. 2. The Army has an interest in maintaining the accuracy of its records for historical purposes. The information in those records must reflect the conditions and circumstances that existed at the time the records were created. 3. The document submitted by the applicant shows she married someone with the last name of Jxxxxx on 25 June 2011, more than 1 year after she was retired. 4. In the absence of compelling information to the contrary, there is no basis for changing the records in this case. 5. A copy of this decisional document, which confirms she married someone with the last name of Jxxxxx, will be filed in the applicant's OMPF. This should serve to clarify any questions or confusion regarding the different names, and adequately document her record. 6. Changes of name based on changes of marital status subsequent to her placement on the Retired List should be submitted to Commander, U.S. Army Human Resources Command, ATTN: AHRC-PDR-V, 1600 Spearhead Division Avenue, Fort Knox, KY 40122.
