THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the unsigned version of Permanent Orders 161-03, dated 10 June 2011, awarding him the Army Good Conduct Medal (2nd Award) from his Official Military Personnel File (OMPF) be removed and replaced by the signed version. 2. He states that the orders for award of the Army Good Conduct Medal (2d Award) without the approval authority's signature only indicate that the award wasn't processed or approved. The award orders were sent by mistake to his records. 3. He provides: * Army Good Conduct Medal (2d Award) orders without signature * Army Good Conduct Medal (2d Award) orders with signature * Email message from Human Resources Command (HRC), Fort Knox, KY CONSIDERATION OF EVIDENCE: 1. The applicant is currently serving on active duty in the rank/grade of specialist/E-4. 2. Permanent Orders 161-03, dated 10 June 2011, awarded the applicant the Army Good Conduct Medal for the period 7 August 2008 through 6 August 2011. These orders were filed in his OMPF and were not signed by the approval authority. 3. A second copy of Permanent Orders 161-03, dated 10 June 2011, awarding him the Army Good Conduct Medal for the period 7 August 2008 through 6 August 2011, were signed by the appropriate approval authority. These orders were not filed in his OMPF. 4. In a 17 August 2011 email message, an HRC representative informed the applicant that they only remove "exact" duplicates [orders]. He was advised that if he wished to have one order removed, he must contact this Board. 5. Army Regulation 600-8-104 (Military Personnel Information Management/ Records), provides in Table 2-1 (Composition of the OMPF) that award orders are filed in the Commendatory and Disciplinary portion of the OMPF. This regulation also provides that once filed such documents become a permanent part of the OMPF and may only be removed by one of several agencies, one of which is the Army Board for Correction of Military Records (ABCMR). DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows the applicant was awarded the Army Good Conduct Medal (2nd Award) by Permanent Orders 161-03, dated 10 June 2011, for the period 7 August 2008 through 6 August 2011. These orders were erroneously filed in the applicant's OMPF without the signature of the approval authority. 2. The applicant provided another copy of Permanent Orders 161-03, dated 10 June 2011, which included the signature of the approval authority. However, these orders were not filed in his OMPF. Therefore, there is a viable reason for removing the unsigned copy of Permanent Orders 161-03, dated 10 June 2011, from the applicant's OMPF. These orders offer no relevant information regarding whether the applicant's award of the Army Good Conduct Medal (2nd Award) was approved. Moreover, removing this erroneous order and replacing it with the signed version would eliminate the potential for confusion to officials authorized to review the performance section of the applicant's OMPF.
