THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) be corrected to show his date of birth (DOB) as December . 2. The applicant states the DOB is incorrectly listed on his DD Form 214. 3. The applicant provides copies of his DD Form 214 and Certificate of Birth, North Carolina showing his DOB as December . CONSIDERATION OF EVIDENCE: 1. The applicant is a retired U.S. Army Reserve (USAR) lieutenant colonel. 2. His records contain numerous documents showing the DOB of December . Among the documents are: * DA Form 61 (Application for Appointment) * SF (Report of Medical Examination), dated 20 July 1954 * DA Form 1140 (Army Reserve Qualification and Availability Statement) * Headquarters, Minnesota Military District assignment orders DA Form 42 (Application for Identification Card * SF , dated 5 March 1973 * DA Form 66 (Officer Qualification Record) * DD Form 214 3. His records contain the following document showing his DOB as December : * TAGD Form 109 (Request for Statement of Service) * DA Form 73 (Certificate of Clearance and/or Security Determination) * AGPZ Form 61 (Computation of Officer's Service) 4. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation directs that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service. It is important that information entered on the form should be complete and accurate. DISCUSSION AND CONCLUSIONS: 1. Although most of his records show the DOB of December , his records do contain documents that indicate the DOB of December , which he claims is correct. 2. In view of the foregoing, there is sufficient evidence for granting the applicant's request to correct his DD Form 214.
