THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service in support of Operation Iraqi Freedom (OIF) from 6 May to 28 August 2009. 2. The applicant states his last deployment to Iraq is not listed on his DD Form 214 because he was separated from the Army shortly after he returned. He previously had other periods of service in Iraq and he was issued a DD Form 215 (Correction to DD Form 214) to show the other periods. 3. The applicant provides his DD Form 214, DD Form 215, and a copy of Permanent Orders 252-04 (Shoulder Sleeve Insignia). CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 25 March 2005 and he held military occupational specialty 11B (Infantryman). He was assigned to the 3rd Battalion, 75th Ranger Regiment and he attained the rank/grade of sergeant/E-5. 2. He was honorably released from active duty on 4 December 2009. He completed 4 years, 8 months, and 10 days of creditable military service. His DD Form 214 shows in: * Item 12f (Foreign Service) - "0000 00 00" * Item 18 (Remarks) does not show service in a hazardous fire/imminent danger pay (HF/IDP) area 3. On 26 August 2011, he was issued a DD Form 215 that corrected: * Item 12f to show completion of 10 months and 11 days of foreign service * Item 18 to show "Service in Iraq from 20060328 - 20060712, 20071004 - 20080109, and 20080630 - 20081019" 4. Permanent Orders 52-04, issued by Headquarters, 3rd Battalion, 75th Ranger Regiment, Fort Benning, GA, on 9 September 2009, awarded him the Shoulder Sleeve Insignia for service in Iraq in support of OIF from 6 May to 28 August 2009. 5. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states for: * block 12f enter the total amount of foreign service completed during the period covered by the DD Form 214 * block 18 for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "Service in (Name of Country Deployed) From YYYYMMDD - YYYYMMDD)" DISCUSSION AND CONCLUSIONS: Permanent orders awarded the applicant the Shoulder Sleeve Insignia for service in Iraq in support of OIF from 6 May 2009 to 28 August 2009, a period of 3 months and 23 days. Therefore, he is entitled to correction of his DD Form 214 to show this foreign service in item 12f and combat service in item 18.
