Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 7b (Home of Record at Time of Entry) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show an address in Pewaukee, Wisconsin. 2. The applicant states: * he was living in Pewaukee, Wisconsin, when he enlisted * he needs his DD Form 214 to reflect his home of record for entitlement to State of Wisconsin veterans' benefits 3. The applicant provides: * DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant's enlistment document shows he enlisted at the Military Entrance Processing Station in Milwaukee, Wisconsin. His home of record is listed as Pewaukee, Wisconsin. 2. He enlisted in the Regular Army on 5 July 2006 for a period of 5 years and served as a military policeman. On 4 July 2011, he was released from active duty and transferred to the U.S. Army Reserve Control Group (Reinforcement) to complete his remaining service obligation. 3. Item 7b of his DD Form 214 shows an address in Newnan, Georgia. 4. Army Regulation 635-5 (Separation Documents) establishes the policies and procedures for completion and distribution of the DD Form 214. It states that a Soldier's initial enlistment contract or appointment document is the source for entering data in item 7a (Place of Entry onto Active Duty) and item 7b. It also states that "home of record" is the place recorded as the home of record of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty. 5. Army Regulation 601-280 (Total Army Retention Program) defines the term "home of record" as the place recorded as the home of the person when he/she entered into the current tour of active duty. DISCUSSION AND CONCLUSIONS: 1. The applicant contends his home of record is Pewaukee, Wisconsin. His enlistment document supports this contention. 2. Since the governing regulation states the enlistment contract is the source for entering data in item 7b and his enlistment document shows his home of record was Pewaukee, Wisconsin, it would be appropriate to correct item 7b of his DD Form 214 to show this address.
