THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his Iraq Campaign Medal and other awards he has received. 2. The applicant states his DD Form 214 does not show the Meritorious Service Medal that was approved after he separated. His DD Form 214 needs to be updated to show he received two Meritorious Service Medals, four Army Commendation Medals, two Army Achievement Medals, two Overseas Service Ribbons, the Iraq Campaign Medal, and the Army Superior Unit Award. 3. The applicant provides a copy of his DD Form 214, DA Form 4037 (Officer Record Brief), and orders for the Army Superior Unit Award. CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a captain in the U.S Army Reserve (USAR) on 12 March 2002. On 4 June 2002, he entered active duty. He was promoted to major/O-4, Medical Corps, on 4 June 2008. 2. His ORB shows in: a. Section VIII (Awards and Decorations) the: * Meritorious Service Medal * Army Commendation Medal (3 Awards) * Army Achievement Medal (2 Awards) * Meritorious Unit Commendation * National Defense Service Medal * ICMSC * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon (2 Awards) b. Section IX (Assignment Information) that he was assigned to Walter Reed Army Medical Center from 28 June 2005 to 10 January 2010. 3. On 31 July 2011, the applicant was honorably discharged. He had completed 9 years, 1 month, and 27 days of creditable active service. 4. The applicant's DD Form 214 shows in: a. item 8a (Last Duty Assignment and Major Command) Walter Reed Army Medical Center; and b. item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized): * Meritorious Service Medal * Army Commendation Medal * Army Achievement Medal * Meritorious Unit Commendation * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon 5. HIs record contains orders for the: a. Meritorious Service Medal for service while assigned as a physician during Operation Iraqi Freedom from 1 April 2008 to 15 February 2009. b. Army Commendation Medal for service while assigned to Tripler Army Medical Center, Hawaii, from 1 July 2002 to 30 June 2005. c. Army Commendation Medal for achievement while assigned to Walter Reed Army Medal Center from 1 July 2005 to 30 June 2007. d. Army Commendation Medal for service and achievement while assigned to Walter Reed Army Medical Center from 1 July 2008 to 1 August 2009. e. Army Achievement Medal for service while assigned to Tripler Army Medical Center, Hawaii, from 1 January to 30 June 2005 f. Army Achievement Medal for achievement as a semi-finalist for the 2007 Bailey K. Ashford Research Award while assigned to Walter Reed Army Medical Center from 1 May 2006 to 1 May 2007. 6. In support of his request, he provides a copy of Permanent Orders 206-13, U.S. Army Human Resources Command, Fort Knox, KY, dated 25 July 2011, showing the Army Superior Service Medal was awarded to Walter Reed Army Medical Center for the period 1 January 2010 to 31 December 2010. 7. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). A bronze service star is authorized for wear with this medal for participation in each credited campaign. During his service in Iraq participation credit was awarded for the following two campaigns: * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-31 August 2010) 8. Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  It provides that Item 18 will contain the following entry for Regular Army Soldiers: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The applicant's ORB shows he was awarded three Army Commendation Medals, two Army Achievement Medals, two Overseas Service Ribbons, and the Iraq Campaign Medal. These awards should be added to his DD Form 214. 2. Orders show he is eligible for the Army Superior Unit Award. His DD Form 214 should be corrected. 3. Records show the applicant participated in two campaign periods during his service in Iraq. Therefore, he is entitled to two bronze service stars for wear on his already-awarded Iraq Campaign Medal. 4. The available evidence shows the applicant was deployed in support of Operation Iraqi Freedom from 1 April 2008 to 15 February 2009. Therefore, this period of service should be shown in Item 18 of his DD Form 214. 5. There is no evidence of record showing the applicant received an additional Meritorious Service Medal or a 4th Army Commendation Medal. His DD Form 214 should not be corrected to show these medals. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting the following awards from item 13 of his DD Form 214: * Army Commendation Medal * Army Achievement Medal * Overseas Service Ribbon b. adding the following awards to item 13 of his DD Form 214: * Army Commendation Medal (3rd Award) * Army Achievement Medal (2nd Award) * Iraq Campaign Medal with two bronze service stars * Overseas Service Ribbon (2nd Award) * Army Superior Unit Award c. adding the following entry to item 18 of his DD Form 214: * SERVICE IN IRAQ FROM 20080401-20091207 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.
