THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, item 4a (Grade, Rate or Rank) and item 4b (Pay Grade) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect the rank and pay grade of private (PV2)/E2. 2. She states she shipped on 19 April 2011 and was promoted when she arrived at basic training. She added that she was never demoted. 3. She provides the following: * DD Form 214 * DA Form 4187 (Personnel Action), dated 21 April 2011 * Leave and Earnings Statement (LES), dated 1-31 May 2011 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in Regular Army on 19 April 2011. She was discharged from active duty on 2 August 2011 under the provisions of Army Regulation 635-200 (Personnel Separations - Active Duty Enlisted Administrative Separations), chapter 11, Entry Level Performance and Conduct, with an uncharacterized description of service. She was credited with 3 months and 14 days of active duty service. 2. Her DD Form 214 lists her rank and pay grade in items 4a and 4b as PV1 and E-1, respectively. The effective date of pay grade in item 12i is listed as 19 April 2011. 3. Her record is void of any documents that show she was demoted. 4. Her DA Form 4187, dated 21 April 2011, show she was promoted to E-2. 5. Additionally, her LES show her pay grade as E-2 with a pay date listed of 19 April 2011. 6. The Total Army Personnel Database - Reserve (TAPDB-R) lists her grade as PV2. Her date of rank is listed as 19 April 2011. 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214. The regulation stipulates that the rank and pay grade held on the date of separation will be entered in items 4a and 4b. DISCUSSION AND CONCLUSIONS: The information obtained from the TAPDB-R confirms the applicant was promoted to PV2/E-2 on 19 April 2011 and her record is void of any documents that show she was demoted. Therefore, PV2/E-2 is the rank and pay grade she held on the date of her separation and her DD Form 214 should be corrected accordingly.
