Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. This regulation states that for item 12 (Record of Service), use extreme care in completing this block since post-service benefits, final pay, retirement credit, and so forth are based on this information. It further states that for item 12d (Total Prior Active Service), enter the total amount of prior active military service less time lost, if any, from previously-issued DD Forms 214. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 for the period ending 1 November 2011 should be corrected to show he completed 3 years, 3 months, and 8 days of total prior active service. 2. The evidence shows he had active duty service not covered by a DD Form 214. Therefore, the U.S. Army Human Resources Command (HRC) should audit his military records and make the necessary adjustments to his active duty service on his DD Forms 214 as appropriate. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by having HRC audit his military records and make adjustments to his active duty service on his DD Forms 214 as appropriate. 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to amending his DD Form 214 for the period ending 1 November 2011 to show he completed 3 years, 3 months, and 8 days of prior active service at this time. ___________x______________ CHAIRPERSON I certify that herein is recorded the true and complete record of the proceedings of the Army Board for Correction of Military Records in this case.
