THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the Defense Superior Service Medal and Army Staff Identification Badge be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states the documentation was submitted to his personnel office but was never added to his Officer Record Brief or permanent record. 3. The applicant provides: * Award certificate, citation, and orders for the Defense Superior Service Medal * Amended orders, dated 18 September 2009 * Certificate for the Army Staff Identification Badge, dated 5 August 2010 CONSIDERATION OF EVIDENCE: 1. The applicant retired in the rank of colonel on 30 September 2011 after completing over 26 years of creditable active service. 2. His DD Form 214 does not show the Defense Superior Service Medal or Army Staff Identification Badge as authorized awards. 3. Permanent Orders, dated 23 June 2009, show he received the Defense Superior Service Medal. 4. He provided a certificate for the Army Staff Identification Badge, dated 5 August 2010. 5. Army Regulation 600-8-22 (Military Awards) states that the Army Staff Identification Badge was first proposed by General Douglas MacArthur on 28 December 1931 to recognize outstanding performance of duty while detailed to the Army General Staff. The badge is awarded by Principal Officials of Headquarters, Department of the Army to military personnel serving on their staff and within their subordinate agencies. Permanent issue must be authorized by Principal Officials with the certificate of authorization constituting authority for wearing the badge as a permanent part of the uniform. DISCUSSION AND CONCLUSIONS: 1. Permanent Orders show the applicant received the Defense Superior Service Medal. Therefore, his DD Form 214 should be corrected to show this medal. 2. The Army Staff Identification Badge certificate provided by the applicant is accepted as sufficient evidence on which to base adding this badge to his DD Form 214.
