THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the NATO Medal and the Army Good Conduct Medal. 2. The applicant states he was awarded the NATO Medal for a deployment in Afghanistan and the Army Good Conduct Medal for 3 years of honorable service. 3. The applicant provides certificates awarding him the NATO Medal and the Army Good Conduct Medal. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 24 September 2008 and he held military occupational specialty 11B (Infantryman). He served in Afghanistan from 4 June 2010 to 28 May 2011. 2. He was honorably released from active duty on 27 January 2012. His DD Form 214 does not show the NATO Medal or the Army Good Conduct Medal. 3. Permanent Orders Number 290-030, issued by Headquarters, 2nd Brigade, 101st Airborne Division, Fort Campbell, KY, on 17 October 2011 awarded him the Army Good Conduct Medal for exemplary behavior, efficiency, and fidelity in active Federal service during the period 24 September 2008 to 23 September 2011. 4. He provides a certificate that shows he was awarded the NATO Medal for service with NATO in relation to operations in Afghanistan during the period 6 June 2010 to 6 June 2011. DISCUSSION AND CONCLUSIONS: The applicant was awarded the NATO Medal and the Army Good Conduct Medal. Neither award is listed on his DD Form 214. Therefore, his DD Form 214 should be corrected to show these awards.
