THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her last name as "M____" instead of "N____." 2. The applicant states she changed her name. 3. The applicant provides copies of her: * social security card showing her last name as "M____" * marriage certificate and marriage license * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 8 January 2009. Her DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States), dated 8 January 2009, shows she enlisted under the last name of "N____." 2. She provided a marriage license showing she was married on 27 January 2012. Her spouse's last name is shown as "M____." 3. She provided her social security card issued on 22 February 2012 which shows her last name as "M____." 4. She was honorably released from active duty on 1 March 2012. The last name shown on her DD Form 214 is "N____." 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. DISCUSSION AND CONCLUSIONS: 1. Her record shows she enlisted, served, and was separated under the last name of "N____." 2. She got married prior to her separation and her spouse's last name is shown as "M____." However, there is no available evidence showing she requested to change her last name while she was serving on active duty. 3. The Army has an interest in maintaining the integrity of its records for historical purposes. The information in those records must reflect the conditions and circumstances that existed at the time the records were created. 4. Regulatory policy does not provide for entering the applicant's married last name in item 1 (Name) of her DD Form 214. However, since she did marry while serving on active duty, it would be appropriate to amend item 18 (Remarks) of her DD Form 214 to show her married name. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.
