IN THE CASE OF: BOARD DATE: 18 September 2012 DOCKET NUMBER: AR20120005626 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 August 2011 to show a second award of the Bronze Star Medal (BSM). 2. The applicant states he was awarded the BSM (2nd Award) after his retirement. He didn't know he was going to receive this second award for his tour of duty in Afghanistan (October 2011 to April 2012). His first award of the BSM for service in Iraq is properly documented. He discovered he had been awarded the BSM (2nd Award) when it arrived at his house in the mail. He would like it added to his DD Form 214. 3. The applicant provides copies of his DD Form 214 and BSM Certificate. CONSIDERATION OF EVIDENCE: 1. With prior enlisted service, the applicant was commissioned a second lieutenant on and entered active duty on 28 September 1989. He served in Iraq from 21 February 2004 to 1 October 2004 and in Afghanistan from 4 October 2010 to 4 April 2011. 2. He was honorably retired in the rank of lieutenant colonel on 31 August 2011. His DD Form 214 for the period ending 31 August 2011 lists one BSM. 3. He provides a BSM Certificate, dated 27 September 2011, awarded for exceptionally meritorious achievement in support of Operation Enduring Freedom for the period 6 October 2010 to 2 April 2011. The certificate shows the award was issued by Permanent Order Number 270-026. DISCUSSION AND CONCLUSIONS: Permanent orders awarded the applicant the BSM (2nd Award) on 27 September 2011. Therefore, he is entitled to correction of his DD Form 214 for the period ending 31 August 2011 to show this award.
