THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his name shown on his interactive Personnel Electronic Records Management System (iPERMS) record and his Honorable Discharge Certificate. 2. The applicant states he was discharged in May 2002 and his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 13 May 2002 correctly shows his first name as G------y, his middle name as O-----g, and his last name as O---o. When he was commissioned as a Reserve officer his DA Form 71 (Oath of Office – Military Personnel) shows the correct names. However, his iPERMS record shows his first name as O-----g, his middle name as O---o, and his last name as G------y. 3. The applicant provides: * DA Form 71 * DD Form 214 for the period ending 10 March 2009 * his Honorable Discharge Certificate, dated 17 January 2012 CONSIDERATION OF EVIDENCE: 1. He was commissioned a second lieutenant in the U.S. Army Reserve (USAR) on 27 February 2007. He had previously served 4 years of active service in an enlisted status in the Regular Army. a. His DD Form 214 for the period ending 13 May 2002 shows his last name as O---o, his first name as G------y, and his middle name as O-----g. b. His DA Form 71, dated 27 February 2007, shows his last name as O---o, his first name as G------y, and his middle name as O-----g. 2. He served a period of active duty for training from 4 January through 10 March 2009. His DD Form 214 for this period of service shows his last name as O---o, his first name as G------y, and his middle name as O-----g. 3. His iPERMS record identifies his last name as G------y, first name as O-----g, and middle name as O---o. 4. His Honorable Discharge Certificate, dated 17 January 2010, shows the name O-----g O---o G------y (first, middle, last name). DISCUSSION AND CONCLUSIONS: 1. He is identified incorrectly in the iPERMS database. He is identified with his first name as his last name, his middle name as his first name, and his last name as his middle name. A Soldier's record on iPERMS should reflect in order, last name, first name, and middle name. Therefore, it would be appropriate at this time to correct his name on iPERMS as shown on his DD Form 214 for the period ending 10 March 2009. 2. His Honorable Discharge Certificate clearly reflects his name incorrectly. It should be entered on the certificate as first name, middle name, last name. Therefore, it is appropriate to reissue his discharge certificate with his correct name.
