THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that the records her deceased husband, a former service member (FSM), be corrected to show his Social Security Number (SSN) as ending in .

2.  The applicant states his records incorrectly show a SSN ending in .

3.  The applicant provides copies of the FSM's military identification card, Medicare Card, and death certificate.

CONSIDERATION OF EVIDENCE:

1.  At the time the FSM initially entered active duty the military utilized the Military Service Number and not the SSN for identification.  The earliest recorded use of an SSN, his security clearance application, utilitizes the SSN ending .

3.  The FSM's records show the use of both the SSN ending in  and .

4.  His DD Form 214 (Report of Transfer or Discharge) utilizes the SSN ending .

5.  The documents provided by the applicant show the FSM's SSN as ending .


DISCUSSION AND CONCLUSIONS:

The preponderance of evidence shows the FSM's SSN was incorrectly shown on several of his service documents including his DD Form 214.  It is appropriate to correct the FSM's records to show the SSN ending in .
