THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the Global War on Terrorism Service Medal (GWOTSM) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 29 October 2011. 2. The applicant states: * while deployed to Iraq in 2007/2008 he was awarded the GWOTSM * Fort Benning, GA, officials failed to put the award on his DD Form 214 * he has a Statement of Service indicating he is entitled to the GWOTSM * his DD Form 214 does not reflect this award 3. The applicant provides his: * Statement of Service * DD Form 214 for the period ending 29 October 2011 CONSIDERATION OF EVIDENCE: 1. The applicant was ordered to active duty from the U.S. Army Reserve on 6 July 2011 in support of Operation New Dawn. He served as a dentist in Iraq from 16 July 2010 to 14 October 2011. He was released from active duty on 29 October 2011. 2. His DD Form 214 for the period ending 29 October 2011 does not show the GWOTSM as an authorized award. 3. He provides a Statement of Service which states he is entitled to wear the GWOTSM. 4. Army Regulation 600-8-22 (Military Awards) states the GWOTSM is authorized for award to members of the Armed Forces of the United States who have participated in GWOT operations outside of the areas of eligibility (AOE) designated for award of the Global War on Terrorism Expeditionary Medal, Afghanistan Campaign Medal, or Iraq Campaign Medal. All Soldiers on active duty, including Reserve Component Soldiers mobilized or National Guard Soldiers activated, on or after 11 September 2001 to a date to be determined having served 30 consecutive days or 60 nonconsecutive days are authorized the GWOTSM. DISCUSSION AND CONCLUSIONS: 1. Notwithstanding the Statement of Service provided by the applicant which shows he is entitled to wear the GWOTSM, he did not serve a period of qualifying service (30 days outside AOE) for award of the GWOTSM. 2. The evidence shows: * he was ordered to active duty on 6 July 2011 and he arrived in Iraq on 16 July 2011 (10 days outside the AOE) * he departed Iraq on 15 October 2011 and he was released from active duty on 26 October 2011 (15 days outside of the AOE) * the total number of days outside of the AOE equals 25 days 3. Therefore, there is insufficient evidence on which to base adding the GWOTSM to his DD Form 214.
