IN THE CASE OF: BOARD DATE: 17 September 2013 DOCKET NUMBER: AR20130001860 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant executed an oath of office on 28 December 2005 as a second lieutenant. On 6 January 2006, he was ordered to active duty. His area of concentration was 12A (Engineer, General). 2. His DD Form 214 shows he completed 6 years, 4 months, and 10 days of creditable active service and does not show award of the JSCM. 5. The applicant provides a copy of his JSCM Certificate which shows the medal was awarded by General Order Number D7-1288-12. 6. Army Regulation 635-5 (Separation Documents) establishes standardized policy for preparing and distributing the DD Form 214. The DD Form 214 provides a clear-cut record of active service at the time of release from active duty, retirement or discharge. It further states to list awards and decorations for all periods of service in priority sequence as specified in Army Regulation 600-8-22 and that each entry will be verified by the Soldier's record.
