THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of two Army Commendation Medals. 2. He states although he was awarded two Army Commendation Medals, his DD Form 214 only reflects one. 3. The applicant provides his DD Form 214 and two DA Forms 4980-14 (Army Commendation Medal Certificate). CONSIDERATION OF EVIDENCE: 1. Following a period of service in the Army National Guard, the applicant is currently serving as a first lieutenant in a U.S. Army Reserve Troop Program Unit. 2. On 25 March 2009, he was ordered to active duty for operational support. He served in Afghanistan from 21 April 2010 through 25 April 2011. 3. On 23 March 2012, he was honorably released from active duty. The DD Form 214 issued to him at the time shows he was awarded or authorized the: * Afghanistan Campaign Medal with one Campaign Star * Army Commendation Medal * Army Achievement Medal * Army Reserve Components Achievement Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * NATO Medal 4. The applicant's record contains, and he also provides: a. a DA Form 4980-14 that shows he was awarded the Army Commendation Medal for meritorious service while serving as the Squadron Battlefield Intelligence Coordinator during Operation Enduring Freedom in Afghanistan from 23 April 2010 to 23 April 2011 by Permanent Orders Number 350-291, issued by Task Force Bastogne, Forward Operating Base Fenty, Jalalabad, Afghanistan, dated 16 December 2010. The standard name line on this certificate shows he was assigned to Headquarters and Headquarters Troop, 1st Squadron, 32nd Cavalry Regiment, 1st Brigade Combat Team. b. a DA Form 4980-14 that shows he was awarded the Army Commendation Medal for meritorious service while serving as the Assistant Intelligence Officer from 16 April 2009 to 1 March 2012 by Permanent Orders Number 023-103, issued by Headquarters, 1st Brigade Combat Team, Fort Campbell, KY, dated 23 January 2012. The standard name line on this certificate shows he was assigned to Headquarters and Headquarters Troop, 1st Squadron, 32nd Cavalry Regiment, 1st Brigade Combat Team. 5. His record also contains U.S. Army Human Resources Command, Fort Knox, KY, Permanent Orders 168-04, dated 17 June 2011, which show the Meritorious Unit Commendation was awarded to Headquarters and Headquarters Troop, 1st Squadron, 32nd Cavalry Regiment, 1st Brigade Combat Team for exceptionally meritorious service in support of Operation Enduring Freedom during the period 24 May 2010 to 23 April 2011. 6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. In establishes standardized policy for the preparation of the DD Form 214. In pertinent part, it states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states for item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22. DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that his DD Form 214 should be corrected to show award of two Army Commendation Medals was carefully considered and determined to have merit. 2. Evidence clearly shows he was awarded two Army Commendation Medals by permanent orders. Therefore, his DD Form 214 should be corrected to show these awards. 3. Evidence also shows his unit was awarded the Meritorious Unit Commendation for accomplishments during his tenure. Therefore, his DD Form 214 should be corrected to show this award.
