THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the spelling of her first and middle name as shown on her birth certificate. 2. She states her name is misspelled on all her service documents, and finance orders show a misspelling of her name. Her birth certificate reflects the spelling of her first and middle name as S--cy M----ll. 3. She provides her birth certificate. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 22 October 1990. Her DD Form 4 (Enlistment/Reenlistment Document) shows the spelling of her first name as S--cey. She signed the document indicating her first name as S--cey. 2. Her DD Form 1966 (Record of Military Processing - Armed Forces of the United States) shows in: a. Section VIIIa (Statement of Name for Official Records) she indicated the name shown on her birth certificate as S--cy M----ll E----. b. Section VIIIb (Name as shown on Social Security Number Card), the name as shown on her social security number card as S--cey M. E----. c. Section VIIIc, she stated that she had not changed her name through any court or other legal procedure and that she preferred to use the name of S--cey M. E---- and she signed the document using this name. 3. A review of her service record revealed documents showing the spelling of her first name both as S--cey and S--cy. The remaining documents in her record consistently show the spelling of her middle name as M----lle. 4. On 27 August 2012, she was honorably retired from active duty by reason of permanent disability. 5. Item 1 (Name (Last, First, Middle)) of the DD Form 214 she was issued at the time shows the spelling of her first name as "S--cey" and her middle name as "M----lle." She digitally signed this discharge document with the spelling of her first name as "S--cey" and her middle name as "M----lle." 6. She provided a copy of her birth certificate showing her first and middle name as "S--cy" M----ll." DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that her name is misspelled on her service documents and finance orders is acknowledged and determined to have merit. 2. Although the applicant indicated on her enlistment contract that she preferred to use the enlistment name of "S--cey" on her official military records, her service record contains documents that show the spelling of her first name both as "S--cey" and "S--cy." Her enlistment contract shows the spelling of her middle name as "M---ll." 3. Therefore, it would be appropriate at this time to correct her DD Form 214 to show the spelling of her first and middle name as it is listed on her birth certificate.
