THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 18 (Remarks) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her service in Honduras from 29 July 2003 through 16 August 2004. 2. The applicant states: * her DD Form 214 erroneously shows her service in Honduras from 29 July 2003 through 16 July 2994 * two 9's were inadvertently typed in instead of two 0's and 07 was typed in instead of 08 (2994 versus 2004 and July versus August) * she did not notice the error until she applied for a job and was told she could not get service credit due to the mistake 3. The applicant provides copies of her DD Form 214 and her DA Form 4037 (Officer Record Brief). CONSIDERATION OF EVIDENCE: 1. On 31 May 1986, the applicant accepted an appointment as a Reserve commissioned officer in the rank of second lieutenant. Upon acceptance of her appointment, she was ordered to active duty with temporary duty beginning on 4 January 1987. 2. The applicant retired from the Regular Army in the rank of lieutenant colonel on 31 July 2012 after completing 25 years, 11 months, and 6 days of total active service. 3. The applicant's DA Form 4037 shows she served in Honduras for 12 months with an ending date of 16 July 2004. Her DD Form 214 shows her service in Honduras from 29 July 2003 through 16 July 2994. 4. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. For an active duty Soldier deployed with his or her unit during their continuous period of active service, enter "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The applicant's contentions have been noted and her supporting documentation has been considered. 2. While the available evidence shows she departed Honduras in 2004, it does not show she departed in August 2004 as she contends. 3. Her DA Form 4037 shows she served in Honduras for 12 months with an ending date of 16 July 2004. Her DD Form 214 should be corrected to show the proper date of her departure from Honduras. The Board further determined the evidence presented was insufficient to warrant a portion of the requested relief.
