THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show item 7b (Home of Record (HOR) at Time of Entry) as Cxxxxx Rxxx 2XX, Lot 7XX, Mountain View, WY, vice 1XXX E 4XXX S, Ogden, UT. 2. The applicant states her HOR on 22 April 2010 was Cxxxxx Rxxx 2XX, Lot 7XX, Mountain View, WY. She enlisted in Utah as that was the only recruiter she could get in contact with but her recruiter put in the wrong address. Her address was not 1XXX E 4XXX S, Ogden, UT. 3. The applicant provides: * her DD Form 214 * three orders, dated 30 August 2012, 6 November 2012, and 8 November 2012 * a high school diploma * three Wyoming Driver License CONSIDERATION OF EVIDENCE: 1. The applicant was born on XX June 1991. She enlisted in the U.S. Army Reserve (USAR) on 21 April 2010 at the Salt Lake City Military Entrance Processing Station (MEPS), Salt Lake City, UT. Item 3 (HOR) of her DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) contains the entry 1XXX E 4XXX S, Ogden, UT. 2. In conjunction with her enlistment in the USAR, she completed a Standard Form 86 (Security Clearance Application), on 21 April 2010, wherein it shows in item 11 ((Where You Have Lived) she listed in 11(1) and 112), respectively, her residence from: * September 2009 to present as 1XXX E 4XXX S, Ogden, UT * July 1991 to August 2009 as 1XXX Wxxxxxx Dxxxx, Green River, WY 3. Item 12 (Where You Went to School) of this Standard Form 86 shows she listed that she attended school from September 2009 to present at Weber State University, 1001 University Circle, Ogden, UT. 4. She was ordered to active duty as a member of her USAR unit in support of Operation Enduring Freedom (OEF) and she entered active duty on 28 September 2011. She was honorably released from active duty on 28 July 2012 to the control of her USAR unit. 5. Item 7b of the DD Form 214 she was issued contains the entry 1XXX E 4XXX S, Ogden, UT. Item 19a (Mailing Address After Separation) contains the entry Cxxxxx Rxxx 2XX, Lot 7XX, Mountain View, WY. 6. Army Regulation 601-280 (Army Retention Program) prescribes the criteria for the Army retention program. Table 11-1 of this regulation defines the HOR as the place recorded as the home of the individual when commissioned, appointed, enlisted, inducted, or ordered into the relevant tour of active duty. The place recorded as the home of the individual when reinstated, reappointed, or reenlisted remains the same as that recorded when commissioned, appointed, enlisted, or inducted or ordered into the relevant tour of active duty unless there is a break in service of more than one full day. Only if a break in service exceeds 1 full day can the HOR be changed by the member. 7. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is prepared for all personnel at the time of retirement, discharge, or release from active duty. Item 7b shows the street, city, state and ZIP code listed as the Soldier’s HOR. The "HOR" is the place recorded as the HOR of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty. DISCUSSION AND CONCLUSIONS: 1. The evidence of record confirms the applicant had been a lifelong resident of Green River, WY, from the time of birth to August 2009 and she was temporarily residing in Ogden, UT, at the time of enlistment in the USAR. 2. When she enlisted in the USAR on 21 April 2010 she listed her residence from July 1991 to August 2009 as 1XXX Wxxxxxx Dxxxx, Green River, WY. Therefore, it would be appropriate to correct her DD Form 214 for the period ending 28 July 2012 to reflect this address as her HOR. 3. With respect to correcting her DD Form 214 to show her HOR as Cxxxxx Rxxx 2XX, Lot 7XX, Mountain View, WY, this is the address she listed as her address after separation from active duty. It was not the address she listed as her residence from July 1991 to August 2009. Therefore, she is not entitled to full relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the entry in item 7b of her DD Form 214 and replacing it with the entry shown in item 11(2) of her Standard Form 86. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
