THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests removal of a DA Form 638 (Recommendation for Award) for award of the Joint Service Achievement Medal and an Army Reserve Component Overseas Training Ribbon consideration memorandum from his Army Military Human Resource Record (AMHRR). 2. The applicant states he was erroneously awarded the Joint Service Achievement Medal and Army Reserve Components Overseas Training Ribbon. A review of his AMHRR by the U.S. Army Human Resources Command (HRC) revealed the error of his ineligibility for these awards by regulation. HRC issued a DD Form 215 (Correction to DD Form 214 – Certificate of Release or Discharge from Active Duty) removing these awards from his DD Form 214 for the period ending 29 October 2012. Now he would like the supporting documents for these awards removed from his AMHRR. 3. The applicant provides copies of an Army Reserve Component Overseas Training Ribbon consideration memorandum, a DA Form 638, his DD Form 214 for the period ending 29 October 2012, and his DD Form 215. CONSIDERATION OF EVIDENCE: 1. The applicant entered active duty in support of Operation Enduring Freedom as a member of the U.S. Army Reserve on 30 September 2011. 2. He provided copies of the following documents in support of his application: * memorandum from the 1st Schools Battalion, 7th Warrior Training Brigade, dated 6 August 2012, subject: Army Reserve Components Overseas Training Ribbon, recommending him for award of the Army Reserve Components Training Ribbon * DA Form 638, dated 9 September 2012, recommending him for award of the Joint Service Achievement Medal and showing this award was approved by Headquarters, Base Support Battalion-North, Permanent Order Number 263-05, dated 22 September 2012 2. He was released from active duty to the control of his U.S. Army Reserve unit on 29 October 2012. His DD Form 214 shows he was awarded or authorized the Joint Service Achievement Medal and Army Reserve Components Training Ribbon. 3. On 2 January 2014, HRC issued a DD Form 215 deleting the Army Reserve Component Overseas Training Ribbon and Joint Service Achievement Medal from his DD Form 214 for the period ending 29 October 2012. 4. A review of his AMHRR revealed copies of the contested documents. 5. Army Regulation 600-8-104 (Army Military Human Resource Records Management) prescribes Army policy for the creation, utilization, administration, maintenance, and disposition of the AMHRR. This regulation states that once a document is placed in the AMHRR it becomes a permanent part of that file and will not be removed from that file or moved to another part of the file unless directed by the proper authorities listed in the regulation. DISCUSSION AND CONCLUSIONS: The evidence of record shows the Joint Service Achievement Medal and Army Reserve Components Training Ribbon were deleted from the applicant's 2012 DD Form 214 for the period ending 29 October 2012. Therefore, he is entitled to correction of his AMHRR by removing the DA Form 638 for award of the Joint Service Achievement Medal and the Army Reserve Component Overseas Training Ribbon consideration memorandum.
