THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of her DD Form 214 for the period ending 3 March 2014 to show award of the: * Army Achievement Medal (4th Award) * North Atlantic Treaty Organization (NATO) Medal (2nd Award) 2. The applicant states: * support documentation was not available at the time her DD Form 214 was prepared * she previously received a NATO Medal for Operation Joint Endeavor-Bosnia for the period 1996 through 1997 3. The applicant provides: * DD Form 214 for the period ending 3 March 2014 * NATO Medal Certificate for the period 31 August 2013 to 25 January 2014 * Army Achievement Medal Certificate for the period 1 to 11 May 2008 CONSIDERATION OF EVIDENCE: 1. The applicant had a prior honorable period of service in the Regular Army from 8 March 1996 to 27 April 2000. 2. Her DD Form 214 for the period ending 27 April 2000 shows, in part, award of the: * Army Achievement Medal (2nd Award) * NATO Medal 3. On 12 December 2007, the applicant enlisted in the U.S. Army Reserve. 4. On 27 March 2013, she was ordered to active duty as a member of the U.S. Army Reserve. 5. 203rd Military Intelligence Battalion Permanent Orders Number 12-127-004, dated 6 May 2012, awarded the applicant the Army Achievement Medal for the period 23 April through 6 May 2012. 6. On 3 March 2014, the applicant was honorably released from active duty. Her DD Form 214 ending 3 March 2014 shows, in part, award of the: * Army Achievement Medal (3rd Award) * NATO Medal 7. The applicant provided a copy of her NATO Medal Certificate for participation in the International Security Assistance Force for the period 31 August 2013 to 25 January 2014 and a copy of her Achievement Medal Certificate for the period 1 to 11 May 2008. 8. Her Enlisted Record Brief, dated 4 March 2014, shows, in part, award of the: * Army Achievement Medal * NATO Medal 9. Army Regulation 600-8-22 (Military Awards) states the NATO Medal was authorized by the Secretary-General of NATO for specific NATO operations. The Secretary of Defense may approve acceptance and wear by U.S. service members who meet the criteria specified by the Secretary-General of NATO. Acceptance of the NATO Medal has been approved for U.S. military personnel who serve under NATO command or operational control in direct support of NATO operations in the former Republic of Yugoslavia from 14 November 1995 through a date to be determined. 10. Army Regulation 600-8-22 states the following missions/operations have been approved by the Secretary of Defense for acceptance and wear of the NATO Medal: (1) operations related to the former Republic of Yugoslavia from 14 November 1995 to a date to be determined, (2) operations related to Kosovo from 13 October 1998 to a date to be determined, (3) Operation Eagle Assist from 12 October 2001 to 16 May 2002), (4) Operation Active Endeavor from 26 October 2001 to a date to be determined, (5) International Security Assistance Force in Afghanistan from 1 June 2003 to a date to be determined, and (6) operations in the Balkans from 1 January 2003 to a date to be determined. 11. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents that must be prepared for Soldiers upon retirement, discharge, or release from active duty service or control of the Active Army. It established standardized policy for preparing and distributing the DD Form 214. Instructions stated to enter awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22. Each entry would be verified by the Soldier's records. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows the applicant was previously awarded the Army Achievement Medal (2nd Award) during her first period of enlistment. She was subsequently awarded the Army Achievement Medal on 9 May 2008 and 6 May 2012. Therefore, she is entitled to correction of her DD Form 214 for the period ending 3 March 2014 to show award of the Army Achievement Medal (4th Award). 2. The applicant was awarded the NATO Medal in a previous enlistment and she later participated in the International Security Assistance Force Operations whereby she was awarded the NATO Medal. 3. Therefore, she is entitled to correction of her DD Form 214 for the period ending 3 March 2014 to show award of the NATO Medal (2nd Award).
