THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 26 March 2014 to show two additional Army Commendation Medals. 2. The applicant states that after he returned from his fourth deployment to Afghanistan and during his redeployment outprocessing at Fort Bliss, TX, he realized he did not have all of his records with him. His DD Form 214 shows he was awarded two awards of the Army Commendation Medal while he was awarded a total of four awards of this medal. 3. The applicant provides three DD Forms 214, nine certificates, an award citation, four DA Forms 638 (Recommendation for Award), two memoranda, and 11 orders. CONSIDERATION OF EVIDENCE: 1. The applicant was serving as a member of the U.S. Army Reserve (USAR) in the rank/grade of major/O-4 when he was ordered to active duty in support of Operation Enduring Freedom (OEF). He entered active duty on 3 December 2011. 2. He was honorably released from active duty on 26 March 2014 to the control of the USAR by reason of completion of required active service. 3. The DD Form 214 he was issued for this period of service shows he served in Afghanistan from 10 December 2011 to 17 November 2013. It also shows, in part, award of the Army Commendation Medal (2nd Award). 4. He provides the following permanent orders (PO): * PO Number 035-1, undated, signed by Brigadier General JMP, U.S. Army, Commanding General, awarding him the Army Commendation Medal for his service in Afghanistan from 19 December 2004 to 15 February 2006 * PO Number 154-22, dated 3 June 2011, issued by, Headquarters (HQ), 2nd Battalion, 34th Infantry Brigade Combat Team (IBCT), awarding him the Army Commendation Medal for his service in Afghanistan from 15 July 2010 to 10 July 2011 * PO Number 198-011, undated, issued by HQ, 37th IBCT, awarding him the Army Commendation Medal for his service in Afghanistan from 3 December 2011 to 2 December 2012 * PO Number 061-007, undated, issued by HQ, Task Force Destiny, awarding him the Army Commendation Medal for his service in Afghanistan from 3 December 2012 to 1 April 2013 DISCUSSION AND CONCLUSIONS: The applicant provides PO that awarded him four awards of the Army Commendation Medal while his DD Form 214 for the period ending 26 March 2014 shows the Army Commendation Medal (2nd Award). Therefore, it would be appropriate to correct this DD Form 214 to show the Army Commendation Medal (4th Award).
