THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her home of record (HOR) as Connecticut (CT). 2. The applicant states her HOR was not updated by her unit. 3. The applicant provides copies of her DD Form 214, ending 11 April 2014 and Orders 13-106-00010, dated 16 April 2013. CONSIDERATION OF EVIDENCE: 1. Having prior service in the U.S. Army Reserve (USAR), the applicant reenlisted in the USAR on 14 December 2011. Her DD Form 4/1 (Enlistment/Reenlistment Document-Armed Forces of the United States) list her HOR as New Haven, CT in item 3 (HOR). 2. She provides Order 13-106-00010, issued by the 412th Theater Engineer Command, Vicksburg, MS, dated 16 April 2013, which ordered her to active duty as a member of the USAR in support of Operation Enduring Freedom (OEF). Her HOR is shown as East Haven, CT on these orders. 3. On 20 April 2013, she entered active duty and she was honorably released from active duty on 11 April 2014 and assigned to her USAR unit. Item 7b of the DD Form 214 she was issued list her HOR as La Porte, TX. 4. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is prepared for all personnel at the time of retirement, discharge, or release from active duty. Item 7b shows the street, city, state and ZIP code listed as the Soldier's HOR. The "HOR" is the place recorded as the HOR of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty. DISCUSSION AND CONCLUSIONS: The evidence of record shows her HOR is listed as East Haven, CT at the time she was ordered to active duty in support of OEF. It is reasonable to presume an administrative error occurred during the time her DD Form 214 was prepared. As a result, her HOR should be corrected as recommended below.
