THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) be amended to show her name as 
M_ _ _ A _ _ _ V _ _ _ F _ _ _ instead of R _ _ _ V _ _ _.

2.  The applicant states her name was legally changed on 21 March 2014.  Her birth certificate shows her name as R _ _ _ V _ _ _, but she is married and she needs to add the last name F _ _ _ to her name.

3.  The applicant provides a DD Form 214 and Change of Name Judgment.  

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Women's Army Corps on 29 March 1962.  Her DD Form 4 (Enlistment Record – Armed Forces of the United States) shows her name as R _ _ _ V _ _ _.

2.  A review of her military record reveals she served under the name R _ _ _     V _ _ _ throughout the entire period of her military service.

3.  She was honorably discharged on 16 March 1965.  Her DD Form 214 shows her name as R _ _ _ V _ _ _.

4.  She provided a Change of Name Judgment that shows her name was changed to M_ _ _ A _ _ _ V _ _ _ F _ _ _ effective 21 March 2014.

5.  Army Regulation 635-5 (Separations Documents), in effect at the time of the applicant's active duty service, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It established standardized policy for the preparation of the DD Form 214.  It stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge.

DISCUSSION AND CONCLUSIONS:

1.  Evidence shows she served under the name of R _ _ _ V _ _ _ throughout her entire period of military service.

2.  For historical purposes, the Army has an interest in maintaining the integrity of its records.  The data and information contained in those records should reflect the conditions and circumstances that existed at the time the records were created.  In the absence of a showing of material error or injustice, there is a reluctance to recommend that those records be changed.  While her desire to now record the name she legally acquired in 2014 in her military records is understandable, there is not a sufficiently compelling reason for compromising the integrity of the Army's records.

3.  The applicant is advised that a copy of this decisional document will be filed in her Official Military Personnel File (OMPF).  This should serve to clarify any questions or confusion in regard to the difference in the name recorded in her military record and to satisfy her desire to have her current legal name change documented in her OMPF.

4.  In view of the foregoing, there is no basis to grant the applicant's requested relief.
