THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 7a (Place of Entry on Active Duty) (PLEAD) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to read "Fort Bliss, Texas." He also requests a new DD Form 214 in lieu of his multiple DD Forms 215 (Correction to DD Form 214). 2. The applicant states he was ordered to report to Fort Bliss, Texas for active duty. 3. The applicant provides copies of his: * active duty orders * DD Form 214 * DD For 215 issued 25 July 2006 * DD Form 215 issued 21 November 2012 * Army Review Boards Agency letter, dated 14 December 2012 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve (USAR) as a Reserve Officers’ Training Corps (ROTC) Cadet on 2 September 1997. 3. A DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) shows his Home of Record as Depew, New York, and his Place of Enlistment/Reenlistment as Buffalo, New York. 4. He was appointed as a USAR Air Defense Artillery second lieutenant on 18 May 2000. 5. Headquarters, U. S. Army Cadet Command, Fort Monroe, Virginia, orders show he was selected for Immediate Active Duty. He was to enter active duty effective 18 May 2000. He was to report on 2 December 2000 to the 21st Adjutant General Replacement Detachment, Fort Hood, Texas, to perform temporary duty at Fort Bliss, Texas, His active duty commitment was for four years. These orders show his address as Depew, New York. 5. He was promoted to captain effective 1 October 2003. 6. The applicant was released from active duty (REFRAD) on 15 March 2005. He had completed 4 years and 10 months of active duty service. His DD Form 214 shows in item 7a: Depew, New York. 7. A DD Form 215, issued 25 July 2006, shows, in part, the entry in item 7a was changed to Buffalo, New York. 8. The Joint Federal Travel Regulations (JFTR), Appendix A (Definitions) defines the PLEAD as the place of acceptance in current enlistment, commission, or appointment of members of the regular Services, or of members of the Reserve components when enlisted, commissioned, or appointed for immediate active duty (emphasis added). This document also notes that the PLEAD changes only if there is a break in service exceeding one full day, in which case it is the place of entry into the new period of service. 9. Army Regulation (AR) 635-5 (Separation Documents), in effect at the time, established the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is prepared for all personnel at the time of their retirement, discharge, or release from active duty. Item 7a of the DD Form 214 shows the place of entry onto active duty. A Soldier’s initial enlistment contract or appointment document is the source for this data. 10. AR 635-5, paragraph 2–7, states once a DD Form 214 has been issued, do not reissue except for the following reasons: a. When directed by proper appellate authority, Executive Order, or by the Secretary of the Army. b. When it is determined that the original DD Form 214 cannot be properly corrected by issuance of a DD Form 215 or if the correction would require issuance of more than two DD Forms 215. DISCUSSION AND CONCLUSIONS: 1. The applicant contends his DD Form 214 should be corrected to show he entered active duty from Fort Bliss, Texas. 2. The applicant's DD Form 4 shows his place of enlistment as Buffalo, New York. 3. The JFTR states the PLEAD as the place of acceptance in current enlistment, commission, or appointment of members of the regular Services, or of members of the Reserve components when enlisted, commissioned, or appointed for immediate active duty (emphasis added). This document also notes that the PLEAD changes only if there is a break in service exceeding one full day, in which case it is the place of entry into the new period of service. 4. There is no record showing the applicant had any break in service. 5. The evidence of record shows his DD Form 214 was corrected by a DD Form 215 to show his PLEAD as Buffalo, New York. 6. There is no reason to change his PLEAD to Fort Bliss, Texas. He did not enter active duty there; he was ordered to perform temporary duty there. 7. There is no correction that would require the issuance of more than two DD Forms 215; therefore reissuance of the DD Form 214 is not necessary. 8. In view of the above, the applicant's request should be denied.
