THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) ending on 4 May 2014 to show the North Atlantic Treaty Organization (NATO) Medal. 2. The applicant states he did not have the certificate when he demobilized in 2014. 3. The applicant provides a certificate awarding him the NATO Medal. CONSIDERATION OF EVIDENCE: 1. Having had prior active service, the applicant was appointed as a Reserve commissioned officer and executed an oath of office on 15 March 1996. He served in a variety of assignments and he was promoted to lieutenant colonel. 2. He was ordered to active duty on 26 April 2013 in support of Operation Enduring Freedom. He served in Afghanistan from 3 May 2013 to 19 April 2014. He was honorably released from active on 4 May 2014. His DD Form 214 does not show the NATO Medal. 3. He provides a certificate awarding him the NATO Medal for service with NATO on operations in relation to the International Security Assistance Force (ISAF) - Afghanistan, from 4 May 2013 to 19 April 2014. 4. Army Regulation 600-8-22 (Military awards) states the NATO Medal is awarded by the Secretary-General of NATO to military and civilian members of the Armed Forces of the United States who participate in designated NATO operations. U.S. military personnel who serve under NATO command or operational control in direct support of certain NATO operations may receive this medal. Service with NATO in relation to International Security Assistance Force in Afghanistan (1 June 2003 to a date to be determined) has been approved by the Secretary of Defense, for acceptance and wear. DISCUSSION AND CONCLUSIONS: He was awarded the NATO Medal for service with NATO in relation to the ISAF-Afghanistan, from 4 May 2013 to 19 April 2014. This award is not shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 to show this award.
