Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests award of the Iraq Campaign Ribbon and inclusion of his period of service in Iraq on his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states these items are not included on his DD Form 214. 3. The applicant provides copies of his DD Form 214, page 1 of a DA Form 638 (Recommendation for Award), and his Combat Infantryman Badge Certificate. CONSIDERATION OF EVIDENCE: 1. The applicant served on active duty in the Regular Army from 21 February 2007 through 6 March 2016. 2. The award recommendation provided shows he served in Iraq from December 2008 through December 2009. 3. 3rd Brigade Combat Team, 25th Infantry Division, Permanent Order Number 024-002, dated 24 January 2013, awarded the applicant the Army Achievement Medal. 4. Defense Finance and Accounting Service records show he received hazardous duty pay/imminent danger pay and combat zone tax exclusion for service in Iraq from 17 December 2008 through 17 December 2009 and service in Afghanistan 1 April 2011 through 1 April 2012. 5. He was honorably released from active duty on 6 March 2013. His DD Form 214 shows in: a. block 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized), award of the Army Commendation Medal (2nd Award), Meritorious Unit Commendation, National Defense Service Medal, Afghanistan Campaign Medal with two campaign stars, Global War on Terrorism Service Medal, Korea Defense Service Medal, Army Service Ribbon, Overseas Service Ribbon (2nd Award), North Atlantic Treaty Organization Medal, Combat Infantryman Badge, Expert Marksmanship Qualification Badge with Rifle Bar, and Army Lapel Button; and b. block 18 (Remarks), the entry "SERVICE IN AFGHANISTAN 20110401-20120401." 6. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. a. The Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). The area of eligibility (AOE) encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 through 31 December 2011. b. A bronze service star is authorized for wear with Iraq Campaign Medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003-1 May 2003) * Transition of Iraq (2 May 2003-28 June 2004) * Iraqi Governance (29 June 2004-15 December 2005) * National Resolution (16 December 2005-9 January 2007) * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-31 August 2010) * New Dawn (1 September 2010-31 December 2011) 7. Army Regulation 635-5 (Separation Documents), in effect at the time, stated to enter "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" for an active duty Soldier deployed with his or her unit during their continuous period of active service. DISCUSSION AND CONCLUSIONS: 1. The applicant is shown to have been deployed on two occasions with only his second deployment being included on his DD Form 214. His DD Form 214 should be corrected to include the statement "SERVICE IN IRAQ FROM 20081217-20091217." 2. The applicant is shown to have served in Iraq during the eligibility period for award of the Iraq Campaign Medal and participated in two campaign periods. His DD Form 214 should be corrected to show award of the Iraq Campaign Medal with two bronze service stars. 3. The applicant's award of the Army Achievement Medal was omitted from his DD Form 214. His DD Form 214 should be corrected to show award of the Army Achievement Medal.
